A large study of 47 countries around the world shows that there are unexplained regional differences in the rate of death and stroke after atrial fibrillation (AF). The investigators conducted a 12-year follow-up analysis of 15 400 patients with AF or atrial flutter from 164 centers in 47 countries from December 2007 to October 2011. A total of 15 361 patients were followed up. A total of 604 patients (4%) developed stroke at 1 year of follow-up, with a total mortality of 11%. Further analysis found that the incidence of stroke and mortality was higher at 1 year of follow-up compared with those who were initially diagnosed as having AF (5% vs.3%, P <0.001 and 16% vs.6% , P <0.001). In addition, AF patients in each region were followed up for stroke at 1 year
