目的讨论金黄色葡萄球菌滤液结合顺铂及深静脉留置针在治疗恶性胸水中的作用。方法对比回顾分析胸腔闭式引流后经深静脉留置针注入金黄色葡萄球菌滤液及顺铂与传统的胸穿抽液后注入顺铂对恶性胸腔积液的疗效。治疗组采用深静脉留置针行胸腔闭式引流,引流胸水后经引流管注入顺铂行胸腔内化疗,每次80mg,金葡液5000-8000单位(10-16支)加生理盐水20Ml,再加顺铂40mg和地塞米松5mg注入胸腔,每周2次,4周为一个疗程。传统组采用传统的穿刺抽液后穿刺针注入顺铂胸腔内化疗。结果治疗组36例患者,完全缓解CR(胸水完全吸收3个月以上)9例,部分缓解PR(胸水减少2/3以上超过1.5个月)21例,有效率83.8%。传统组患者38例,CR 7例,PR例11例,总有效率47.3%。结论胸腔闭式引流短期内较彻底引流胸水是控制恶性胸水成功的关键.彻底引流胸水后深静脉留置针注入金葡液及顺铂能更有效控制胸水生长及白细胞减少,病人胸水控制后使原发病灶进行肿瘤治疗有效率明显提高,预后得到改善。
Objective To investigate the role of Staphylococcus aureus filtrate combined with cisplatin and deep vein indwelling needle in the treatment of malignant pleural effusion. Methods The retrospective analysis of thoracic drainage after thoracic drainage through the deep vein indwelling needle injection of Staphylococcus aureus filtrate and cisplatin with conventional chest fluid pumping cisplatin on malignant pleural effusion. In the treatment group, the deep vein indwelling needle was used for closed thoracic drainage. After drainage of pleural effusion, Cisplatin was infused into the thoracic cavity through the drainage tube. Each time 80mg, 5000-8000 goldmine liquid (10-16) plus normal saline 20Ml Plus cisplatin 40mg and dexamethasone 5mg into the chest, 2 times a week, 4 weeks for a course of treatment. The traditional group used the traditional puncture after puncture liquid injection of cisplatin intrapleural chemotherapy. Results In the 36 cases of treatment group, CR was completely relieved in 9 cases with CR (pleural effusion completely absorbed for more than 3 months), and partial response was relieved in 21 cases with PR (pleural effusion more than 2/3 more than 1.5 months), with an effective rate of 83.8%. The traditional group of 38 patients, CR 7 cases, PR 11 cases, the total effective rate was 47.3%. Conclusion Thoracic drainage is more effective in the control of malignant pleural effusion than thoracic drainage in the short term.Diffusion of pleural effusion after intravenous infusion of pleural fluid and cisplatin can effectively control the growth of pleural effusion and leukopenia, Focal tumor treatment efficiency was significantly improved, the prognosis was improved.
