目的:分析高强度聚焦超声治疗子宫肌瘤的消融特点。方法:回顾性分析我院行高强度聚焦超声治疗子宫肌瘤患者84例,依据患者消融治疗前MRI表现分为T2高信号组,T2低信号组,与T2混杂信号组。依据子宫肌瘤大小分为小肌瘤组、中肌瘤组、大肌瘤组,以及子宫肌瘤的位置分为前壁组、后壁组、侧壁组,比较不同分组患者子宫肌瘤体积消融率。结果:不同信号分组子宫肌瘤体积消融率存在明显差异,T2低信号组>T2混杂信号组>T2高信号组,3组比较差异有统计学意义(P<0.05)。不同大小肌瘤组体积消率差异无统计学意义(P>0.05)。子宫前壁肌瘤体积消融率大于后壁,差异有统计学意义(P<0.05),与侧壁肌瘤体积消融率差异无统计学意义(P>0.05)。结论:高强度聚焦超声消融子宫肌瘤的临床效果取决于子宫肌瘤的位置以及肌瘤内部组织成分,而与肌瘤的大小相关性较小。
Objective: To analyze the ablation characteristics of high intensity focused ultrasound in the treatment of uterine fibroids. Methods: A retrospective analysis of 84 cases of high-intensity focused ultrasound treatment of uterine fibroids in patients with ablation according to the performance of patients before radiotherapy were divided into T2 high signal group, T2 low signal group, and T2 mixed signal group. According to the size of uterine fibroids, small fibroid group, myoma group, large fibroid group, and uterine fibroids were divided into anterior wall group, posterior wall group, and lateral wall group, comparing the different groups of patients with uterine fibroids volume Ablation rate. Results: There was a significant difference in volume ablation rate of uterine fibroids between different signal groups, T2 low signal group> T2 mixed signal group> T2 high signal group. There was significant difference between the three groups (P <0.05). There was no significant difference in volume rate of different sizes fibroids (P> 0.05). The ablation rate of anterior uterine fibroids was larger than that of the posterior wall, the difference was statistically significant (P <0.05). There was no significant difference in volume ablation rate of side wall myoma (P> 0.05). CONCLUSION: The clinical effect of high-intensity focused ultrasound in the treatment of uterine fibroids depends on the location of uterine fibroids and the internal tissue components of fibroids, but not on the size of fibroids.
