背景:原发性胆汁性肝硬化(PBC)是一种自身免疫性肝病,目前发病机制尚未完全阐明。除遗传和环境因素外,自身抗体、多种免疫细胞以及细胞因子共同参与了PBC的发生。近年研究发现白细胞介素-9(IL-9)具有多向免疫调节功能,与多种变态反应性疾病和自身免疫性疾病相关。目的:探讨IL-9在PBC患者中的表达及其临床意义。方法:收集PBC患者血清标本30例,肝穿刺标本20例,另取10名同期健康体检者外周血标本和4例正常肝组织标本作为正常对照组。采用ELISA法检测血清IL-9水平,采用免疫组化染色检测肝组织IL-9表达。分析IL-9水平与血清生化、免疫指标和肝组织病理分期的相关性。结果:PBC患者血清IL-9水平较正常对照组显著升高(P<0.05),并与血清IgG水平呈正相关(r=0.681,P<0.01)。PBC患者肝组织中的IL-9阳性细胞数量较正常对照组显著增加(P<0.01),并与肝组织病理分期呈正相关(rs=0.465,P<0.05)。结论:PBC患者外周血和肝组织中的IL-9均呈高表达,且其表达水平与血清IgG水平和肝组织病理分期相关,提示IL-9可能在PBC的发病过程中发挥重要作用。
BACKGROUND: Primary biliary cirrhosis (PBC) is an autoimmune liver disease and the current pathogenesis has not yet been fully elucidated. In addition to genetic and environmental factors, autoantibodies, immune cells and cytokines are involved in the pathogenesis of PBC. Recent studies have found that interleukin-9 (IL-9) has a multi-directional immune regulatory function, and a variety of allergic diseases and autoimmune diseases. Objective: To investigate the expression of IL-9 in PBC and its clinical significance. Methods: Thirty patients with PBC and 20 patients with liver biopsy were collected. Peripheral blood samples from 10 healthy people and 4 normal liver tissues were collected as normal control group. Serum IL-9 levels were detected by ELISA, and the expression of IL-9 in liver tissue was detected by immunohistochemistry. The correlation between IL-9 level and serum biochemistry, immune index and pathological stage of liver tissue was analyzed. Results: The serum level of IL-9 in PBC patients was significantly higher than that in normal control group (P <0.05), and positively correlated with serum IgG level (r = 0.681, P <0.01). The number of IL-9 positive cells in liver tissue of PBC patients was significantly higher than that of normal control group (P <0.01), and positively correlated with pathological stage of liver tissue (rs = 0.465, P <0.05). Conclusion: The expression of IL-9 in peripheral blood and liver tissue of PBC patients is highly expressed, and its expression level is correlated with serum IgG level and pathological stage of liver tissue, suggesting that IL-9 may play an important role in the pathogenesis of PBC.
