目的探讨单纯鼾症和混合型睡眠呼吸暂停低通气综合征睡眠结构、血氧饱和度(SaO2)、呼吸紊乱指数(AHI)的差异及其这些差异可能的原因。方法分别对30例混合型睡眠呼吸暂停低通气综合征患者和62例单纯型鼾症患者进行多导睡眠监测,包括浅睡眠、深睡眠、快速动眼(REM)睡眠、SaO2、AHI,并将其结果进行对比。结果与单纯型鼾症患者比较,混合型睡眠呼吸暂停低通气综合征组Ⅰ、Ⅱ期睡眠(37.66±19.04、14.05±14.21)增多,Ⅲ、Ⅳ期、REM睡眠(3.31±5.69、0.87±1.37、41.67±23.36)减少;混合型睡眠呼吸暂停低通气综合征组患者的AHI指数(70.89±18.14)升高(P<0.01)、而SaO2减低(66.00±10.48)(P<0.01)。结论对于构成睡眠结构的各期所占之百分比,混合型睡眠呼吸暂停低通气综合征组与单纯型鼾症组比较差异无统计学意义(P>0.05),但前者存在Ⅰ、Ⅱ期睡眠增多和Ⅲ、Ⅳ期、REM睡眠减少的趋势;混合型睡眠呼吸暂停低通气综合征组患者的AHI指数明显升高、而SaO2却明显减低。
Objective To investigate the difference of sleep structure, blood oxygen saturation (SaO2) and respiratory distress index (AHI) in simple snore and mixed sleep apnea hypopnea syndrome and its possible causes. Methods 30 cases of mixed sleep apnea-hypopnea syndrome and 62 cases of simple snore were monitored by polysomnography, including light sleep, deep sleep, REM sleep, SaO2, AHI, The results are compared. Results Compared with patients with simple snore, the sleep in stage Ⅰ and Ⅱ of combined sleep apnea-hypopnea syndrome group increased (37.66 ± 19.04 and 14.05 ± 14.21), stage Ⅲ and Ⅳ, REM sleep (3.31 ± 5.69 and 0.87 ± 1.37 , 41.67 ± 23.36). The AHI (70.89 ± 18.14) and the SaO2 (66.00 ± 10.48) in patients with mixed sleep apnea-hypopnea syndrome were significantly lower than those in patients with mixed sleep apnea-hypopnea syndrome (P <0.01). Conclusions There is no significant difference between sleep apnea / hypopnea syndrome group and simple snore snoring group (P> 0.05), but there is an increase of sleep stage Ⅰ and Ⅱ in the former And Ⅲ and Ⅳ stages, and REM sleep decreased. The AHI index of patients with mixed sleep apnea-hypopnea syndrome was significantly increased, while SaO2 was significantly decreased.
