Phenobarbital is an enzyme inducer that promotes drug metabolism by inducing hepatic drug enzymes. But the role of promoting neostigmine metabolism and excretion in bile is not through the induction of enzymes. The elimination of neostigmine in the body is related to the effect of phenobarbital on bile flow, bile duct transport, and hepatic blood flow. This article is speculative. It is well-known that the metabolism of neostigmine in the body is related to the quaternary amine structure in its molecule, which is mainly hydrolyzed by hepatic enzymes to form 3-hydroxyphenyl 3-methylamine, which is then induced into 3-hydroxyglucuronide.
