目的探讨原发性高血压患者脉搏波传导速度(PWV)改变及相关因素。方法整群选取合肥市某医院2009-09-2010-08间住院的原发性高血压患者共424例,其中男女各223例和201例,年龄(65.6±11.9)岁,采用脉搏波速度测定仪测定颈股动脉脉搏波速度(cfPWV)、颈桡动脉脉搏波速度(crPWV)和颈远端动脉脉搏波传导速度(cdPWV),并以cfPWV>12m/s为动脉僵硬度增高。结果住院高血压患者crPWV、cfPWV和cdPWV为(9.3±2.6)、(10.8±4.4)、(9.9±6.0)m/s,动脉僵硬度增高检出率为34.2%。crPWV、cfPWV和cdPWV无性别差异。cfPWV与收缩压(r=0.236,P<0.01)、年龄(r=0.294,P<0.001)、脉压(r=0.243,P<0.001)呈正相关。crPWV与收缩压(r=0.120,P<0.05)、舒张压(r=0.168,P<0.01)呈正相关。多元线性回归分析显示年龄和收缩压是cfPWV的独立影响因素。结论原发性高血压患者动脉僵硬度异常率较高,cfPWV与年龄、收缩压密切相关。
Objective To investigate the changes of pulse wave velocity (PWV) and its related factors in patients with essential hypertension. Methods A total of 424 patients with Hypertension admitted to a hospital in Hefei City from September 2009 to October 2010 were enrolled in this study. Among them, 223 were male and female and 201 were patients (65.6 ± 11.9 years), respectively. The pulse wave velocity The carotid artery pulse wave velocity (cfPWV), carotid radial artery pulse wave velocity (crPWV) and distal carotid artery pulse wave velocity (cdPWV) were measured and the arterial stiffness increased with cfPWV> 12 m / s. Results The in-hospital hypertension patients had crPWV, cfPWV and cdPWV of (9.3 ± 2.6), (10.8 ± 4.4) and (9.9 ± 6.0) m / s, respectively. The detection rate of arterial stiffness was 34.2%. No gender differences in crPWV, cfPWV and cdPWV. CFPV was positively correlated with systolic blood pressure (r = 0.236, P <0.01), age (r = 0.294, P <0.001) and pulse pressure (r = 0.243, P <0.001). CrPWV was positively correlated with systolic blood pressure (r = 0.120, P <0.05) and diastolic blood pressure (r = 0.168, P <0.01). Multivariate linear regression analysis showed that age and systolic blood pressure were independent factors of cfPWV. Conclusion The abnormal rate of arterial stiffness in patients with essential hypertension is high, cfPWV is closely related to age and systolic blood pressure.
