目的:分析外伤性脉络膜破裂患者的多模式影像图特征。方法:郑州大学第一附属医院2018年2月至2019年7月外伤性脉络膜破裂14例(14只眼)纳入回顾性研究。患眼均行眼底照相、荧光素眼底血管造影(FFA)、吲哚菁绿血管造影(ICGA)及光学相干断层扫描(OCT)检查,分析其不同模式影像的特点。结果:眼底照相中5只眼(35.71%)因视网膜或脉络膜出血而未能发现脉络膜破裂灶,9只眼见后极部视网膜黄白色弧形病灶。其中2只眼脉络膜破裂灶FFA表现为透见荧光,10只眼早期为弱荧光,晚期呈高荧光,2只眼未发现破裂灶。12只眼ICGA表现为条状荧光缺损区,2只眼ICGA始终为低荧光。OCT结果显示2只眼视网膜色素上皮层连续性中断,边缘翘起,其下脉络膜反射条带平整。8只眼示椭圆体带及RPE-脉络膜复合光带粗糙中断并向下凹陷,局部脉络膜隆起或凹陷,信号增强,4只眼因扫描范围受限未能发现破裂灶。结论:外伤性脉络膜破裂患者行FFA联合ICGA检查可全面显示脉络膜破裂灶并可同时判断视盘和视网膜受损情况,为最佳影像检查方法。“,”Objective:To analyze the multi-mode imaging characteristics of traumatic choroidal rupture.Methods:This was a retrospective study. Fourteen eyes of 14 patients with traumatic choroidal rupture from Feb. 2018 to Jul. 2019 in the First Affiliated Hospital of Zhengzhou University were included in the study. Fundus photography, fundus fluorescein angiography (FFA), indocyanine angiography (ICGA) and optical coherence tomography (OCT) were performed to analyze the characteristics under different modes.Results:Fundus photography showed 5 eyes (35.71%, 5/14) hadn’t been detected with choroidal rupture due to retinal or choroidal hemorrhage. And 9 eyes had been detected with yellow-white arcuate lesions in the posterior pole retina. The lesions were single or multiple, curved, strip or irregular shape. FFA images of choroidal rupture showed transmitted fluorescence in 2 eyes. Ten eyes showed weak fluorescence in early stage and high fluorescence in late stage. No lesion was found in two eyes. ICGA showed stripe-shaped fluorescent defect areas in 12 eyes, and low fluorescence in 2 eyes. OCT showed interrupted RPE layer with its edge lifted and choroidal reflection band flat in 2 eyes. Eight eyes showed the ellipsoidal band and RPE-choroidal complex band were roughly interrupted and depressed downward with local choroidal bulge or depression and signal enhanced. Four eyes hadn’t been found with lesions because of limited scanning range.Conclusion:FFA combined with ICGA examination for patients with traumatic choroidal rupture can show all choroidal ruptures and judge damages of optic disc and retina, which is the best imaging method.
