目的 :观察不同时间给予小剂量氟芬合剂预防琥珀胆碱所致肌纤维成束收缩的临床意义。方法 :选择ASAⅠ~Ⅱ级、无神经肌肉功能异常的择期腹部手术病人 15 0例。随机分成 5组 ,每组 30例。 5组病人均依次静注氟芬合剂 (氟哌啶 0 1mg/kg、芬太尼 3μg/kg)、丙泊酚 2mg/kg、琥珀胆碱 1mg/kg。氟芬合剂与琥珀胆碱间隔时间Ⅰ~Ⅴ组分别为 1、 2、 3、 4、 5min ,静注琥珀胆碱前先静注丙泊酚 ,静注琥珀胆碱后仔细观察有无肌纤维成束收缩及其程度。结果 :静注氟芬合剂后 ,各组肌纤维成束收缩都有不同程度减轻 ,但以Ⅲ、Ⅳ、Ⅴ组效果最好。经统计学处理Ⅲ、Ⅳ、Ⅴ组与Ⅰ、Ⅱ组比P <0 0 0 5 ,差异具有非常显著性。Ⅲ、Ⅳ、Ⅴ组间比P >0 0 5 ,差异无显著性。结论 :小剂量氟芬合剂与琥珀胆碱两药注射的时间间隔以 3min为宜。
Objective: To observe the clinical significance of different doses of flufenfen to prevent bundles shrinkage caused by succinylcholine. Methods: Fifty ASA Ⅰ ~ Ⅱ patients with elective abdominal surgery without neuromuscular dysfunction were selected. Randomly divided into 5 groups, 30 cases in each group. All 5 patients were followed by intravenous injection of fenofu mixture (droperidol 0 1mg / kg, fentanyl 3μg / kg), propofol 2mg / kg, succinylcholine 1mg / kg. Fenfen mixture and succinyl choline interval Ⅰ ~ Ⅴ group were 1, 2, 3, 4, 5min, intravenous succinylcholine before intravenous injection of propofol, intravenous injection of succinylcholine after careful observation of muscle fibers into Shrink and its extent. Results: After intravenous fenfen mixture, each group of muscle fibers bundles shrink to varying degrees, but to Ⅲ, Ⅳ, Ⅴ group the best. Statistically, there was a significant difference between group Ⅲ, Ⅳ, Ⅴ and group Ⅰ, Ⅱ (P <0 055). Ⅲ, Ⅳ, Ⅴ group than the P> 0 0 5, the difference was not significant. Conclusion: The injection of flufenoxifene and succinylcholine at a time interval of 3min is advisable.
