Abstract Objective To identify Prostate cancer with high and low Gleason score using diffusion-weighted MRI (DW-MRI). Methodology This prospective study was approved by the Ethics Committee. Eighty-four patients with prostate or bladder cancer scheduled for radical prostatectomy voluntarily participated in the study and performed DW-MRI examinations to calculate the apparent diffusion coefficient (ADC) within the lesion and the irrelevant motions within the voxel [IVIM: Pure diffusion coefficient (Dt), pseudo-diffusivity (FP) and pseudo diffusion coefficient (Dp)] and high b (calculated by Hessian filtering). Based on the Gleason score (≥7 for high scores, ≤6 for low scores) determined by postoperative pathologic analysis of the entire prostate, these parameters (applied alone or in combination as a logistic regression model) were used to identify the lesion grade.
