目的探讨乳腺癌患者注意偏向及心理弹性与创伤后成长(posttraumatic growth,PTG)的关系。方法选取2015年1月—2016年11月接受治疗的乳腺癌患者295例。采用一般情况调查表、注意偏向量表、心理弹性量表和创伤后成长量表对乳腺癌患者进行横断面调查。计量资料比较采用t检验或方差分析,并做Pearson相关分析和多元逐步回归分析。P<0.05为差异有统计学意义。结果乳腺癌患者注意偏向总分为(68.25±11.9 2)分,心理弹性总分为(26.49±8.20)分,PTG总分为(65.50±12.28)分。新的可能性得分最高(5.71±1.46)分,个人力量得分最低(0.82±0.54)分。正性注意偏向与PTG及其各维度均呈正相关(均P<0.05),负性注意偏向与PTG均存在负相关(均P<0.05),心理弹性及各维度与PTG存在正相关(均P<0.05)。不同文化程度、宗教、疾病分期、费用来源、家庭关系乳腺癌患者的PTG总分差异有统计学意义(均P<0.05)。多元逐步回归分析结果显示,费用来源、家庭关系、宗教、积极注意偏向、乐观是乳腺癌患者PTG的影响因素(均P<0.05)。结论乳腺癌患者PTG处于中等稍偏上水平,其注意偏向及心理弹性与创伤后成长有关。
Objective To investigate the relationship between attention bias and psychological resilience and posttraumatic growth (PTG) in patients with breast cancer. Methods 295 breast cancer patients treated from January 2015 to November 2016 were selected. A cross-sectional survey of breast cancer patients was conducted using a general questionnaire, a biased attitude scale, a psycho-elastic scale and a post-traumatic growth scale. Measurement data were compared using t test or analysis of variance, and do Pearson correlation analysis and multiple stepwise regression analysis. P <0.05 for the difference was statistically significant. Results The total score of attention bias in breast cancer patients was (68.25 ± 11.9 2) points, the total score of mental resilience was (26.49 ± 8.20) points and the score of PTG was (65.50 ± 12.28) points. The new odds score the highest (5.71 ± 1.46) points, the lowest personal power score (0.82 ± 0.54) points. Positive attention bias was positively correlated with PTG and its dimensions (all P <0.05), negative attention bias was negatively correlated with PTG (all P <0.05), and psychological resilience and dimensions were positively correlated with PTG <0.05). The PTG total score of breast cancer patients with different educational level, religion, disease stage, cost source and family relationship were significantly different (all P <0.05). Multiple stepwise regression analysis showed that source of expense, family relationships, religion, positive attention bias and optimism were the influencing factors of PTG in breast cancer patients (all P <0.05). Conclusions PTG in patients with breast cancer is at a mild to moderate level, and its biases and psychological resilience are related to posttraumatic growth.
