目的探讨肝细胞肝癌(HCC)神经上皮细胞转化基因1(NET-1)及皮质肌动蛋白表达水平与HCC患者预后的关系。方法用免疫组化SP法检测103例石蜡标本HCC组织及57例癌旁组织中NET-1和皮质肌动蛋白表达,分析其与临床病理特征和生存率之间的关系。结果 HCC组织中,NET-1和皮质肌动蛋白阳性表达率分别为85.96%和73.68%,高于癌旁组织的59.60%和28.10%(P<0.01)。HCC组织NET-1表达与肿瘤大小、分级、TNM分期和肝硬化背景有关(P<0.05或P<0.01)。皮质肌动蛋白表达与肿瘤包膜完整性、脉管转移、TNM分期有关(P<0.05或P<0.01)。HCC中,NET-1和皮质肌动蛋白表达呈显著正相关(r=0.280,P<0.05)。NET-1和皮质肌动蛋白表达均阳性组5年生存率低于两者均阴性组(27.78%vs.54.54%)(P<0.05)。结论 HCC中的NET-1和皮质肌动蛋白表达增加,可作为评价HCC肿瘤分级、分期及判断预后的重要参考指标。
Objective To investigate the relationship between the expression of NET-1 and cortical actin in hepatocellular carcinoma (HCC) and the prognosis of patients with HCC. Methods The expressions of NET-1 and cortical actin in 103 paraffin-embedded HCC tissues and 57 para-cancerous tissues were detected by immunohistochemical SP method. The relationship between the expression of NET-1 and cortical actin was analyzed with clinicopathological features and survival rate. Results The positive rates of NET-1 and cortical actin in HCC tissues were 85.96% and 73.68%, respectively, which were higher than 59.60% and 28.10% (P <0.01) of paracancerous tissues. The expression of NET-1 in HCC tissues was related to tumor size, grading, TNM stage and cirrhosis (P <0.05 or P <0.01). Cortical actin expression was correlated with tumor envelope integrity, vessel metastasis and TNM staging (P <0.05 or P <0.01). There was a significant positive correlation between NET-1 and cortical actin expression in HCC (r = 0.280, P <0.05). The 5-year survival rate of both NET-1 and cortical actin positive group was lower than that of both negative group (27.78% vs.54.54%) (P <0.05). Conclusion The increased expression of NET-1 and cortical actin in HCC can be used as an important reference index to evaluate the grade, stage and prognosis of HCC.
