目的:探讨白细胞介素-8(IL-8)和α-肿瘤坏死因子(TNF-α)在慢性阻塞性肺疾病(COPD)气道炎症中的作用。方法:采用酶联免疫吸附法(ELISA)检测10例COPD急性发作期和10例正常对照组痰及血清的IL-8和TNF-α水平。结果:COPD患者痰IL-8水平(1982.24±901.07)ng/L和TNF-α水平(72.60±43.16)ng/L均显著高于正常人痰IL-8(769.60±570.14)ng/L和TNF-α水平(23.90±18.79)ng/L(P<0.01)。此外,COPD患者痰IL-8水平和TNF-α水平亦分别明显高于其血清水平(P<0.01),且痰IL-8和TNF-α水平与其血清水平均无相关关系(P>0.05)。但是COPD患者痰IL-8水平与FEV1%呈明显负相关(r=-0.67,P<0.01),痰TNF-α水平亦与FEV1%呈一定负相关(r=-0.54,P<0.05)。结论:IL-8和TNF-α在COPD气道炎症及气道阻塞中均起重要作用,检测痰IL-8和TNF-α水平较血清更能直接反映COPD气道炎症情况。
Objective: To investigate the role of interleukin-8 (IL-8) and α-tumor necrosis factor (TNF-α) in airway inflammation in chronic obstructive pulmonary disease (COPD). Methods: The levels of IL-8 and TNF-α in 10 cases of acute exacerbation of COPD and 10 cases of normal control group were detected by enzyme-linked immunosorbent assay (ELISA). Results: The levels of IL-8 in sputum of patients with COPD (1982.24 ± 901.07 ng / L and TNF-α were significantly higher than those of normal sputum IL-8 (769.60 ± 570.14) ng / L and TNF -α level (23.90 ± 18.79) ng / L (P <0.01). In addition, levels of IL-8 and TNF-α in sputum of patients with COPD were significantly higher than those in patients with COPD (P <0.01), and no correlation was found between serum IL-8 and TNF-α levels in sputum (P> 0.05) . However, sputum IL-8 levels in COPD patients were negatively correlated with FEV1% (r = -0.67, P <0.01). There was also a negative correlation between sputum TNF-α and FEV1% (r = -0.54, P <0.05). Conclusion: Both IL-8 and TNF-α play an important role in the airway inflammation and airway obstruction in COPD. The levels of IL-8 and TNF-α in sputum can directly reflect the airway inflammation in COPD.
