目的探讨血清中细胞因子在矽肺发生发展过程中的变化情况。方法选择山东某金矿矽肺Ⅰ、Ⅱ期患者、接尘但无矽肺临床表现的工人各40名分别为矽肺Ⅰ、Ⅱ期组、接尘组,另选40名与其他各组均衡可比的地面后勤人员作为对照组,取入选对象血清,应用酶联免疫吸附法(ELISA)测定血清中结缔组织生长因子(Connective Tissue Growth Factor,CTGF)、Ⅱ型肺泡细胞表面抗原(typeⅡalveolar cell surface antigen,KL-6)、白细胞介素-17(Interleukin,IL-17)、单核细胞趋化因子(Monocyte Chemotactic factor-1,MCP-1)、基质金属蛋白酶-9(Metalloproteinases-9,MMP-9)和血小板衍生因子(Platelet Derived Growth Factor,PDGF)的含量。结果与对照组比,接尘组、矽肺Ⅰ期组和矽肺Ⅱ期组血清中CTGF、KL-6、PDGF含量均高于对照组(P<0.05),矽肺Ⅰ期组和矽肺Ⅱ期组血清中IL-17、MCP-1及MMP-9含量也高于对照组(P<0.05),且矽肺Ⅰ期组血清中IL-17、MCP-1及MMP-9达到最高值(P<0.05);逐步判别法形成Fisher判别函数,该函数的符合率为92.5%(148/160),误判率为7.5%(12/160);该判别函数对对照组、接尘组、矽肺Ⅰ期组及矽肺Ⅱ期组的正确判别符合率均高于90%。结论 CTGF、KL-6、IL-17、MCP-1、MMP-9和PDGF做为矽肺早期监测的生物标志物具有重要意义,通过建立多指标联合的判别函数可筛选出用于矽肺病情分级诊断的细胞因子。
Objective To investigate the changes of serum cytokines during the development of silicosis. Methods Forty-four patients with stage I and II silicosis of silicosis in Shandong Province and those with clinical manifestations of silicosis without silicosis were selected as the silicosis stage I and II groups, followed by the dust exposure group, and another 40 were selected to be balanced with the other groups Logisticians were taken as the control group, and serum of selected subjects was taken.
