目的 观察单肺通气(OLV)对开胸手术患者炎症反应的影响.方法 择期开胸手术患者30例,分为双肺通气(Ⅰ)组和单肺通气(Ⅱ)组,每组15例.Ⅱ组在OLV前1 min(T_1)、结束后3 min(T_2)及其后30 min(T_3),Ⅰ组开胸前1 min(T_1),关胸前3 min(T_2)及其后30 min(T_3)采取颈内静脉血,测定细胞因子及炎性介质的活性.结果 与T_1时比较,两组中性粒细胞在T_2、T_3时显著升高(P<0.05),Ⅱ组超氧化物歧化酶(SOD)在T_2、T_3时显著下降(P<0.05),而丙二醛(MDA)、细胞间黏附分子1(ICAM-1)、肿瘤坏死因子α(TNF-α)显著升高(P<0.05).在T_2、T_3时,Ⅱ组SO)D显著低于Ⅰ组,而MDA、ICAM-1、TNF-α显著高于Ⅰ组(P<0.05).结论 单肺通气较双肺通气更容易引起或加重肺损伤.“,”Objective To observe the effects of one-lung ventilation(OLV) on systemic inflammatory responses in patients undergoing thoracic surgery.Methods Thirty patients undergoing thoracic surgery were randomly divided into two groups of OLV and two lung ventilation(TLV) with 15 cases each.Blood samples of jugular vein were taken 1 min before OLV or chest opening in groupTLV(T_1) ,at 3 min after completion of OLV in group OLV or before chest closing in group TLV (T_2), and 30 min after T_2(T_3) for detecting serum levels of neutrophil, superoxide dismutase(SOD),malondialdehyde(MDA), intercellular adhesion molecule-1(ICAM-1), and tumor necrosis factor-α(TNF-α).Results Compaered to those at T_1, serum neutrophil was higher at T_2 and T_3 in both groups(P<0.05).Serum SOD was lower and MDA,ICAM-1 and TNF-α were higher at T_2 and T_3 in group OLV than those at T_2( P<0.05).Serum SOD was lower and MDA,ICAM-1 and TNF-α were higher in group OLV than those in group TLV at T_2 and T_3(P<0.05).Conclusion OLV has harmful effect on lung function more than TLV.
