目的探讨核磁共振成像(MRI)对非典型脑膜瘤的诊断价值。方法回顾分析绵阳市中心医院2011年1月—2015年1月收治的非典型脑膜瘤患者18例,均行MRI平扫、液体衰减反转恢复序列(Fl AIR)、DWI及增强扫描,对患者的肿瘤位置、大小、形态、瘤周水肿、瘤内囊变坏死及出血、瘤体信号及强化特点进行观察分析。结果所有患者中,肿瘤位于大脑镰旁及矢状窦、大脑凸面10例,鞍旁5例,桥小脑角区3例;类圆形14例,不规则3例,分叶状1例;平扫:T1WI低信号10例,TIWI等信号6例,呈稍高信号2例;T2WI呈稍高信号10例,T2WI呈高信号5例,等高混杂信号3例;增强扫描:不均匀强化11例,中、轻度不均匀强化4例,均匀强化3例;12例可见脑膜尾征。结论非典型脑膜瘤MRI影像学表现不典型,需根据肿瘤部位、信号及强化等特点,提高其诊断准确率。
Objective To investigate the diagnostic value of magnetic resonance imaging (MRI) for atypical meningioma. Methods 18 cases of atypical meningiomas admitted from January 2011 to January 2015 in Mianyang Central Hospital were analyzed retrospectively. All patients underwent MRI plain scan, FL AIR, DWI and contrast-enhanced scan. The tumor location, size, shape, peritumoral edema, tumor necrosis and hemorrhage, tumor signals and enhanced features were observed and analyzed. Results In all patients, the tumors were located in the parasite and sagittal sinus of the brain. There were 10 cases of convex cerebellum, 5 cases of parasellar and 3 cases of cerebellopontine angle. There were 14 cases of round type, 3 cases of irregular shape and 1 case of lobulated type. : T1WI low signal in 10 cases, TIWI signal in 6 cases, showed a slightly higher signal in 2 cases; T2WI was slightly higher signal in 10 cases, T2WI showed high signal in 5 cases, 3 cases of mixed signal; enhanced scan: uneven enhancement in 11 cases , 4 cases of moderate and mild non-uniform enhancement, 3 cases of uniform enhancement; 12 cases of meningeal tail signs can be seen. Conclusion The manifestations of atypical meningiomas are not typical. According to the characteristics of tumor location, signal and enhancement, the accuracy of diagnosis should be improved.
