根据纳入排除标准检索文献,应用QUADAS打分评价文献质量,采用Meta-disc软件对敏感性、特异性等进行异质性检验和合并分析,绘制SROC曲线,最终纳入17篇文献评价99mTc-MDP骨显像与CT诊断乳腺癌转移的效能。以患者为单位,99mTc-MDP骨显像和CT合并的敏感度(SEN)、特异度(SPE)、阳性似然比(LR+)、阴性似然比(LR-)、诊断比值比(DOR)、Q*、SROC曲线下面积分别为0.87和0.99、0.81和0.98、3.88和13.86、0.2和0.03、27.73和612.17、0.8418和0.9732、0.9097和0.9952;以病灶为单位,99mTc-MDP骨显像合并的SEN、SPE、LR+、LR-、DOR、Q*、SROC曲线下面积分别为0.86、0.97、13.32、0.16、102.4、0.8944、0.9528。本文提示:无论是99mTc-MDP骨显像还是CT对乳腺癌骨转移都具有较高的诊断效能。
According to the inclusion criteria, the quality of the literature was evaluated using QUADAS scoring, and the heterogeneity of sensitivity and specificity was examined and analyzed by Meta-disc software. The SROC curve was drawn. Finally, 17 articles were included in the evaluation of 99mTc-MDP bone- Like and CT diagnosis of breast cancer metastasis. Sensitivity (SEN), Specificity (SPE), Positive Likelihood Ratio (LR +), Negative Likelihood Ratio (LR-) and Diagnostic Odds Ratio (DOR) were calculated in patients with 99mTc- , Q * and SROC curves were 0.87 and 0.99, 0.81 and 0.98, 3.88 and 13.86, 0.2 and 0.03, 27.73 and 612.17, 0.8418 and 0.9732, 0.9097 and 0.9952, respectively; with lesion as 99mTc-MDP bone imaging The areas under the curve of SEN, SPE, LR +, LR-, DOR, Q * and SROC were 0.86, 0.97, 13.32, 0.16, 102.4, 0.8944 and 0.9528, respectively. This article suggests that both 99mTc-MDP bone imaging and CT have high diagnostic efficacy for breast cancer bone metastases.
