目的:利用股骨滑车与胫骨结节的水平距离(TTTG)比较前交叉韧带单束重建术与双束重建术在恢复膝关节旋转稳定性方面的差异。方法:回顾分析2012年9月至2013年6月间在本院就诊并手术的前交叉韧带断裂患者66例。其中,单束重建组45例,男30例,女15例,年龄28.9±8.0岁;双束重建组21例,男16例,女5例,年龄24.9±7.8岁。术前利用MRI横断面测量TTTG值,术后利用CT横断面进行TTTG值测量。结果:两组患者的年龄和性别均无统计学差别。单束重建组中,TTTG值从术前的10.1±3.2 mm增加到术后的13.4±3.7 mm,差异存在统计学意义。双束重建组中,TTTG值从术前的9.4±2.7 mm增加到术后的13.6±3.0 mm,差异也存在统计学意义。单束重建组与双束重建组相比较,TTTG值增大绝对值的差异无统计学意义,TTTG值增大率的差异也无统计学意义。结论:TTTG值可用以反映前交叉韧带重建术后膝关节旋转功能的改善。单束重建术和双束重建术改善膝关节旋转功能的作用类似。
OBJECTIVE: To compare the difference in the rotation stability of knee joint between single-bundle anterior cruciate ligament reconstruction and double-bundle reconstruction using the horizontal distance between the trochanter and the tibial tubercle (TTTG). Methods: A retrospective analysis of 66 cases of anterior cruciate ligament rupture in our hospital from September 2012 to June 2013 was performed. Among them, 45 cases were single-bundle reconstruction, 30 males and 15 females, with a mean age of 28.9 ± 8.0 years old. There were 21 males and 16 females with 5 females, aged 24.9 ± 7.8 years. TTTG values were measured preoperatively by MRI and TTTG measurements by CT postoperatively. Results: There was no significant difference in age and gender between the two groups. In the single bundle reconstruction group, the TTTG value increased from 10.1 ± 3.2 mm preoperatively to 13.4 ± 3.7 mm postoperatively, with statistical significance. In the double-bundle reconstruction group, the TTTG value increased from 9.4 ± 2.7 mm before surgery to 13.6 ± 3.0 mm after surgery, and the difference was statistically significant. Compared with the double-bundle reconstruction group, there was no significant difference in the absolute value of TTTG value between the single-bundle reconstruction group and the double-bundle reconstruction group, and there was no significant difference in the increase rate of TTTG value. Conclusion: The TTTG value can be used to reflect the improvement of knee rotation function after anterior cruciate ligament reconstruction. The effects of single beam reconstruction and double beam reconstruction on knee rotation are similar.
