目的探讨血液灌流(HP)在急性重症有机磷农药中毒(AOPP)治疗中的应用价值。方法入选病例为2006年4月—2008年6月在滨州医学院附属医院住院的重度AOPP患者38例,按入院后是否接受血液灌流治疗分为灌流组(HP组)和非灌流组(非HP组),非HP组按常规予积极洗胃、导泻、催吐,并予阿托品解磷定治疗。HP组在上述治疗基础上同时加用HP治疗,观察2组住院时间、ChE恢复时间、病程中阿托品的用量及中间综合征(IMS)发生率。通过对HP组与非HP组上述指标的比较,判断HP与非HP治疗重度AOPP患者的疗效。结果HP组住院时间为(7.5±2.3)d,非HP组住院时间为(10.5±3.1)d,2组比较有显著性差异(P<0.05);ChE恢复时间HP组为(5.5±1.4)d,非HP组为(8.3±2.5)d,2组比较有显著性差异(P<0.05);阿托品用量HP组为(355.3±88.0)mg,非HP组为(527.5±79.0)mg,2组比较有极显著性差异(P<0.01);IMS发生率HP组为5%,非HP组为22%,2组比较有极显著性差异(P<0.01)。结论HP治疗重度AOPP可减少患者住院时间,缩短患者ChE恢复时间,减少阿托品的用量,降低IMS征的发生率,因此,HP在治疗AOPP中有显著疗效。
Objective To investigate the value of hemoperfusion (HP) in the treatment of acute severe organophosphorus pesticide poisoning (AOPP). Methods A total of 38 patients with severe AOPP who were hospitalized at Affiliated Hospital of Binzhou Medical College from April 2006 to June 2008 were enrolled in this study. They were divided into perfusion group (HP group) and non-HP group Group), non-HP group according to routine active gastric lavage, catharsis, vomiting, and to atropine phosphate treatment. The HP group was treated with HP on the basis of the above treatment. The length of hospital stay, the recovery time of ChE, the amount of atropine in the course of the disease and the incidence of intermediate syndrome (IMS) were observed. HP and non-HP treatment of severe AOPP patients by comparing the above indicators of HP group and non-HP group. Results The length of hospital stay in HP group was (7.5 ± 2.3) d, that in non-HP group was (10.5 ± 3.1) days, and there was significant difference between two groups (P <0.05) (8.3 ± 2.5) d in the non-HP group, and there was significant difference between the two groups (P <0.05). The atropine dosage was 355.3 ± 88.0 mg in HP group and 527.5 ± 79.0 mg in non-HP group (P <0.01). The incidence of IMS was 5% in HP group and 22% in non-HP group, there was a significant difference between the two groups (P <0.01). Conclusion HP treatment of severe AOPP can reduce hospitalization time, shorten the recovery time of patients with ChE, reduce the amount of atropine and reduce the incidence of IMS signs, therefore, HP has a significant effect in the treatment of AOPP.
