目的 观察MC+型椎间融合器有效内固定长度对脊髓型颈椎病患者早期融合率的影响.方法 回顾性研究2012年9月至2016年2月河南省人民医院脊柱脊髓诊疗中心收治的102例(单节段86例,双节段16例)脊髓型颈椎病患者,均接受颈前路椎间盘切除减压植骨融合术治疗,共植入118个MC+型椎间融合器,分别测量各患者术后内固定嵌片的有效内固定长度(Effective Internal Fixed Length,EIFL定义为内固定嵌片远端与下位椎体上终板平面的垂直距离),根据EIFL的不同,分为a、b、c三组,记录术前、术后7天、3个月、6个月的VAS评分、JOA评分,统计分析各组术后3、6个月植骨融合率.结果 手术前后VAS、JOA评分差异有统计学意义(P<0.05),术后3个月各组植骨融合率有差异(P<0.05),术后6个月随访患者均达到骨性融合,各组差异无统计学意义(P>0.05).结论 MC+融合器治疗脊髓型颈椎病疗效确切;术中尽可能延长内固定嵌片的有效内固定长度,可增加患者早期骨性融合率.“,”Objective To observe the effect of effective internal fixation length of MC + intervertebral fusion cage on early fusion rate of patients with cervical spondylotic myelopathy.Methods A retrospective analysis of 102 patients (86 cases of single segment fusion and 16 cases of double-segment fusion) in patients with cervical spondylotic myelopathy who underwent spinal cord treatment center from September 2012 to February 2016 in Henan Province People's Hospital.A total of 118 MC + interbody fusion devices were implanted.All patients underwent anterior cervical disc decompression and bone graft fusion and internal fixation.The effective internal fixation length of the internal fixation inserts was measured respectively (The effective internal fixation length (EIFL) was defined as the vertical distance between the distal end of the internal fixation panel and the plane of the endplate on the lower vertebral body).According to the effective internal fixation length,divided into a,b,c three groups.The JOA and VAS scores were recorded preoperative,1 week,3 months and 6 months after operation.The fusion rates were observed at 3 months and 6 months after operation in each group and analyzed statistically.Results The scores of postoperative JOA and VAS were significantly lower than those before operation (P < 0.05).The difference of fusion rate between the three groups was statistically significant (P < 0.05).All patients underwent bony fusion at 6 months postoperatively.There was no significant difference between each groups (P > 0.05).Conclusions MC + fusion device can improve the symptoms of patients with cervical spondylotic myelopathy.The effective internal fixation length of internal fixation can increase the rate of early bone fusion.
