背景:肺癌患者因抑郁、肺功能降低、自觉体力下降、疲乏、精力不济等影响生活质量;如出现并发症或晚期恶病质将影响患者生存期。目的:研究p53,p27,bcl-2三种基因蛋白与非小细胞肺癌病理特征的关系。设计:以病理标本为研究对象的对照实验研究。单位:一所大学医院呼吸科和一所大学中心实验室。对象:1997-06/2002-12手术切除非小细胞肺癌组织标本76例,均为原发性肺癌,且未接受任何治疗方法应用SP免疫组化法检测76例手术切除非小细胞肺癌标本中三种基因蛋白的表达。主要观察指标:p53,p27,bcl-2的阳性表达情况。结果:76例标本中28例(37%,28/76)p53过度表达,34例(45%,34/76)p27过度表达,37例bcl-2过度表达(49%,37/76),7例患者p53,p27,bcl-2均过度表达。p53阳性表达率随分化程度降低而升高,bcl-2和p27阳性表达率随分化程度降低而降低,且高、低分化组间差异均有显著性意义(P<0.05)。但是三种蛋白阳性表达率均与肺癌组织学类型、淋巴结转移及病理分期无明显关系(P>0.05)。结论:p53,p27和bcl-2基因过度表达可能与非小细胞肺癌的发生、发展有关。
BACKGROUND: Patients with lung cancer suffer from depression, reduced lung function, decreased physical strength, fatigue, and lack of energy, affecting quality of life; if complications or late cachexia affect patient survival. OBJECTIVE: To study the relationship between p53, p27 and bcl-2 gene proteins and pathological features of non-small cell lung cancer. Design: A controlled experimental study of pathological specimens. Units: A university hospital respiratory department and a university central laboratory. PARTICIPANTS: A total of 76 cases of non-small cell lung cancer (NSCLC) were surgically resected from June 1997 to December 2002. All of them were primary lung cancers. No treatment was used. Seventy-six patients with non-small cell lung cancer (NSCLC) were examined by SP immunohistochemistry. Expression of three gene proteins. MAIN OUTCOME MEASURES: Positive expression of p53, p27 and bcl-2. RESULTS: Of the 76 specimens, 28 (37%, 28/76) had p53 overexpression, 34 (45%, 34/76) p27 overexpressed, and 37 had bcl-2 overexpression (49%, 37/76). In 7 patients, p53, p27, and bcl-2 were overexpressed. The positive expression rate of p53 increased with the degree of differentiation, and the positive rate of bcl-2 and p27 decreased with the degree of differentiation, and there was a significant difference between high and poor differentiation groups (P<0.05). However, the positive expression rates of the three proteins were not significantly associated with histological type, lymph node metastasis, and pathological stage (P>0.05). Conclusion: Overexpression of p53, p27 and bcl-2 genes may be related to the occurrence and development of non-small cell lung cancer.
