目的研究大肠腺瘤和腺癌组织中与张力蛋白和辅助蛋白同源的磷脂酶mRNA(PTENmRNA)、缺氧诱导因子-1αmRNA(HIF-1αmRNA)、血管内皮生长因子(VEGF)蛋白的表达及其临床意义。方法PTENmRNA、HIF-1αmRNA检测采用原位杂交技术,应用免疫组织化学方法检测VEGF蛋白的表达。结果HIF-1αmRNA阳性表达为棕黄色杂交颗粒,主要定位于肿瘤组织坏死边缘区域的肿瘤细胞胞浆中。62例大肠腺癌标本中,PTENmRNA、HIF-1mRNA、VEGF蛋白阳性表达率分别为51.6%、67.7%、59.7%。18例大肠腺瘤标本中,PTENmRNA、HIF-1mR-NA、VEGF蛋白阳性表达率分别为77.8%、44.4%、33.3%。大肠腺癌VEGF蛋白阳性表达率显著高于大肠腺瘤(P<0.05),而PTENmRNA则相反。有局部复发、淋巴结转移、肝转移、Dukes分期高的大肠腺癌组织中HIF-1mRNA阳性表达率显著增高,而PTENmRNA表达则相反。HIF-1α与VEGF呈显著正相关(χ2=4.751,P<0.05);HIF-1αmRNA表达与PTENmRNA呈显著负相关(χ2=21.84,P<0.01);HIF-1αmRNA与VEGF无相关性(χ2=2.597,P>0.05)。结论抑癌基因PTEN的低表达或突变缺失诱导HIF-1α及其靶基因VEGF的过度表达在大肠腺瘤-腺癌发展过程中起重要作用。
Objective To study the expression of phospholipase mRNA (PTENmRNA), hypoxia inducible factor-1αmRNA (HIF-1αmRNA) and vascular endothelial growth factor (VEGF) protein homologous to tensin and accessory protein in colorectal adenoma and adenocarcinoma Clinical significance. Methods PTENmRNA and HIF-1αmRNA were detected by in situ hybridization, and immunohistochemistry was used to detect the expression of VEGF protein. Results The positive expression of HIF-1αmRNA was brown-yellow hybrid particles, which mainly localized in the cytoplasm of tumor cells in the marginal area of tumor necrosis. The positive rates of PTENmRNA, HIF-1mRNA and VEGF in 62 cases of colorectal adenocarcinoma were 51.6%, 67.7% and 59.7% respectively. The positive rates of PTENmRNA, HIF-1mR-NA and VEGF in 18 specimens of colorectal adenoma were 77.8%, 44.4% and 33.3%, respectively. The positive rate of VEGF protein in colorectal adenocarcinoma was significantly higher than that in colorectal adenoma (P <0.05), while the expression of PTEN mRNA was opposite. The positive rate of HIF-1mRNA in local recurrence, lymph node metastasis, liver metastasis and Dukes staging was significantly higher than that in PTENmRNA. There was a significant positive correlation between HIF-1α and VEGF (χ2 = 4.751, P <0.05); HIF-1αmRNA was negatively correlated with PTENmRNA (χ2 = 21.84, 2.597, P> 0.05). Conclusions The overexpression of HIF-1α and its target gene VEGF induced by the low expression or mutation of tumor suppressor gene PTEN plays an important role in the development of colorectal adenoma-adenocarcinoma.
