目的探讨剪切波弹性成像在不同大小颈部淋巴结良恶性鉴别中的诊断价值。方法对80例患者共125个颈部淋巴结进行常规超声及剪切波弹性成像检查,根据淋巴结长径分为3组(组I:≤1cm,组Ⅱ:1~2cm,组Ⅲ:>2cm),以手术切除后获得的病理结果为诊断金标准。结果 3组中良恶性淋巴结最大弹性模量值之间的差异具有统计学意义,组I和组Ⅲ的约登指数和阳性似然比大于组Ⅱ,组I和组Ⅲ的阴性似然比小于组Ⅱ,组I的特异度最高。结论最大弹性模量值对长径≤1cm和长径>2cm淋巴结的诊断价值>1~2cm的淋巴结,对于长径≤1cm的淋巴结的特异度更高。有望对转移性淋巴结的早期筛查提供帮助。
Objective To investigate the diagnostic value of shear wave elastography in differentiating benign and malignant cervical lymph nodes of different sizes. Methods A total of 125 cervical lymph nodes of 80 patients were examined by conventional ultrasound and shear wave elastography. The lymph nodes were divided into 3 groups (group I: ≤1cm, group Ⅱ: 1-2cm, group Ⅲ:> 2cm) , To obtain the pathological results after surgical excision for the gold standard. Results The difference between the maximum elastic modulus of benign and malignant lymph nodes in three groups was statistically significant. The Youden index and positive likelihood ratio of group I and group III were greater than that of group II, and the negative likelihood ratios of group I and group III were less than Group II, group I the highest specificity. Conclusions The maximal elastic modulus values of lymph nodes> 1 ~ 2 cm in diagnosis of lymph nodes with long diameter ≤1 cm and long diameter> 2 cm are higher than those with long diameter ≤1 cm. It is expected to help early screening of metastatic lymph nodes.
