目的:报告Herbert螺钉治疗掌指关节内骨折的临床疗效。方法:自2015年6月至2018年5月，我们共收治25例掌指关节内骨折患者。其中闭合骨折5例，开放性骨折20例，近节指骨基底骨折18例，掌骨头骨折7例，骨折均累及关节面，均采用Herbert螺钉固定，术后2周开始进行早期功能锻炼。结果:术后伤口均Ⅰ期愈合，随访时间为4～26个月，平均12个月。骨折愈合时间为8～14周，平均10.5周。无骨髓炎发生，无明显创伤性关节炎表现，关节功能恢复良好。根据中华医学会手外科学会上肢部分功能评定试用标准评定疗效：优12例，良8例，中5例，优良率为80.0%。结论:应用Herbert螺钉固定治疗掌指关节内撕脱骨折，操作方法简单，骨折固定牢固，疗效可靠。“,”Objective:To report the clinical efficacy of Herbert screw in the treatment of intraarticular fracture of metacarpophalangeal joints.Methods:From June 2015 to May 2018, 25 patients with intraarticular fracture of metacarpophalangeal joints, including 5 closed fractures, 20 open fractures, 18 proximal phalangeal basal fractures, 7 metacarpal head fractures, all involving the articular surface, were all fixed with Herbert screw. Early functional exercise began 2 weeks after operation.Results:All the wound achieved primary healing. The follow-up period ranged from 4 to 26 months with an average of 12 months. The fracture healing time was 8 to 14 weeks with an average of 10.5 weeks. There was no osteomyelitis, no obvious traumatic arthritis, and the joint function recovered well. According to the evaluation criteria of upper limb function issued by Hand Surgery Society of Chinese Medical Association, the results were rated as excellent in 12 cases, good in 8 cases, fair in 5 cases with the excellent and good rate being 80.0%.Conclusion:Herbert screw fixation is a simple and effective method for the treatment of intraarticular avulsed fracture of metacarpophalangeal joints.
