目的探讨改良低半卧位在全髋关节置换术后护理中的安全性和有效性。方法将100例全髋关节置换术后患者随机分为平卧位组和改良低半卧位组。主要观察指标为Kolcaba的舒适状况量表(General Comfort Questionnaire,GCQ)、术后视觉模拟评分(VAS)、呕吐、误吸、假体脱位、压疮和肺部感染等。结果与平卧位相比,改良低半卧位患者术后1h、3h和6h GCQ评分显著增加(P<0.05);术后1h、3h和6h VAS评分均有不同程度的减低,但差异无统计学意义(P>0.05)。二组患者术后呕吐、肺部感染和住院时间差异无统计学意义(P>0.05),无一例发生误吸、假体脱位和压疮等并发症。结论改良低半卧位在全髋关节置换术后6h内是安全有效的,值得进一步推广。
Objective To investigate the safety and efficacy of modified low semi-recumbent postoperative care in total hip arthroplasty. Methods 100 patients after total hip arthroplasty were randomly divided into supine group and modified low semi-supine group. MAIN OUTCOME MEASURES: Kolcaba’s General Comfort Questionnaire (GCQ), postoperative visual analog scale (VAS), vomiting, aspiration, prosthesis dislocation, pressure ulcers and lung infections. Results Compared with the supine position, the GCQ scores at 1h, 3h and 6h after operation in the patients with modified lower half-lying position were significantly increased (P <0.05). The VAS scores at 1h, 3h and 6h after operation were decreased to some extent, but the difference was not statistically significant Significance (P> 0.05). There was no significant difference in postoperative vomiting, pulmonary infection and hospital stay between the two groups (P> 0.05). No complications such as aspiration, prosthesis dislocation and pressure sore were found in the two groups. Conclusion The modified low semi-recumbent position is safe and effective within 6 hours after total hip arthroplasty and is worth further promotion.
