目的:探讨彩色多普勒超声(CDU)对于动脉硬化闭塞症(ASO)患者病变程度分级判断与内皮素(ET)、一氧化氮(NO)水平变化的规律及其临床意义。方法:应用CDU将90例ASO患者根据闭塞节段数量、狭窄程度及侧支数量的不同分为轻、中、重度3组,每组30例;20例经CDU行双下肢动脉检查正常者为对照组,同时检测其血浆ET及血清NO水平。结果:ASO组血清NO水平低于对照组(P<0.01),血浆ET水平高于对照组(P<0.01),其中重度ASO患者血浆ET水平高于轻度、中度组(P<0.01),血清NO水平低于轻度、中度组(P<0.01);中度ASO组患者血浆ET水平高于轻度组(P<0.01),血清NO水平低于轻度组(P<0.01);轻度组ASO患者血浆ET水平高于正常对照组(P<0.01),血清NO水平低于对照组(P<0.01)。结论:CDU对ASO的分级判断与血ET和NO变化程度具有一定关联性,该分级方法可以较详尽地判定ASO的病变程度,为临床提供更合理的量化指标。
OBJECTIVE: To investigate the change of severity of lesions and the changes of endothelin (ET) and nitric oxide (NO) in patients with arteriosclerosis obliterans (ASO) by color Doppler ultrasound (CDU) and its clinical significance. Methods: Ninety patients with ASO were divided into mild, moderate and severe groups according to the number of occlusion segments, the degree of stenosis and the number of collateral vessels by CDU. Thirty patients in each group were treated by CDU. Twenty patients with normal lower limbs were examined by CDU The control group, at the same time test its plasma ET and serum NO levels. Results: The level of serum NO in ASO group was lower than that in control group (P <0.01), and the plasma ET level was higher in control group (P <0.01) (P <0.01). The level of plasma ET in moderate ASO group was higher than that in mild group (P <0.01), while the level of NO in serum was lower than that in mild group (P <0.01) (P <0.01). The level of serum ET in mild ASO patients was lower than that in control group (P <0.01). Conclusion: The classification of ASO by CDU is correlated with the degree of changes of blood ET and NO. The classification method can determine the severity of ASO in detail and provide a more reasonable quantitative index for clinical practice.
