目的:与传统10g尼龙丝记录法及神经肌电图检测中神经传导速度检测比较,探讨改良10g尼龙丝记录法(即与足面感觉对比记录法)对查糖尿病周围神经病变的有效性。方法:设神经传导速度检测结果特异性为100.0%,分别采用改良10g尼龙丝记录法(改良法)及传统10g尼龙丝记录法(传统法)检查88例2型糖尿病患者,通过比较改良法及传统法在周围神经病变诊断方面的敏感性及特异性。结果:与神经传导速度比较,改良法与传统法的敏感性分别为(94.4%,20.4%),特异性分别为(85.3%,76.5%),与神经传导速度检测的Kappa值分别为(0.806,-0.027),提示改良法在敏感性及特异性方面均高于传统法,差异有统计学意义(P<0.01)。结论:改良10g尼龙丝记录法可明显提高10g尼龙丝检测方法的敏感性与特异性。
OBJECTIVE: To compare the effectiveness of 10g nylon wire recording method (that is, comparing with the recording of sensory footpad) to detect peripheral neuropathy of diabetes mellitus in comparison with traditional 10g nylon wire recording method and nerve conduction velocity test in neuromuscular examination. Methods: The specificity of nerve conduction velocity test was 100.0%, 88 patients with type 2 diabetes mellitus were examined by modified 10g nylon filament recording method (modified method) and traditional 10g nylon filament recording method (traditional method) respectively. Sensitivity and specificity of traditional method in the diagnosis of peripheral neuropathy. Results: Compared with the nerve conduction velocity, the sensitivities of the modified method and the traditional method were (94.4%, 20.4%) and (85.3%, 76.5%) respectively, and the Kappa values with the nerve conduction velocity were (0.806 , -0.027), suggesting that the improved method in the sensitivity and specificity were higher than the traditional method, the difference was statistically significant (P <0.01). Conclusion: The improved 10g nylon yarn recording method can significantly improve the sensitivity and specificity of 10g nylon yarn detection method.
