目的探讨氟代脱氧葡萄糖正电子发射计算机断层显像(18F-FDG PET/CT)检查对骨结核诊断的价值。方法回顾性分析5例确诊为骨结核患者的18 F-FDG PET/CT图像,采用定性和半定量分析方法,结合葡萄糖摄取形态及CT影像学特点,综合判断病灶部位、累及范围及各部位病变性质。结果 5例骨结核可分为单发型、多发型和弥漫型,在PET/CT图像上特征性表现为环形或不均匀葡萄糖摄取增高、周围伴冷脓肿并出现钙化,或骨破坏区有明显沙砾样死骨。其中,弥漫型显示骨髓广泛性浸润(无明显骨破坏),与血行播散有关。结论骨结核18 F-FDG PET/CT显像有一定特点,对全身性结核的诊断和与恶性肿瘤鉴别诊断时有较大价值。
Objective To investigate the diagnostic value of fluorodeoxyglucose positron emission tomography (18F-FDG PET / CT) in the diagnosis of bone tuberculosis. Methods We retrospectively analyzed 18 F-FDG PET / CT images of 5 patients diagnosed as having bone tuberculosis. By using qualitative and semi-quantitative analysis methods, combined with the characteristics of glucose uptake and CT imaging, we comprehensively evaluated the lesion, nature. Results Five cases of tuberculosis could be divided into single type, multiple type and diffuse type. The characteristic features of PET / CT images were annular or uneven glucose uptake, peripheral cold abscess with calcification, or obvious gravel in the bone destruction area Like the dead bones. Among them, diffuse type showed extensive bone marrow infiltration (no significant bone destruction), and the spread of blood related. Conclusion 18 F-FDG PET / CT imaging of bone tuberculosis has certain characteristics and is of great value in the diagnosis of systemic tuberculosis and in the differential diagnosis of malignant tumors.
