目的评价颞下颌关节紊乱病(TMD)病人的口腔相关生活质量,并探讨心理因素与TMD的关系。方法选取新疆医科大学第一附属医院、新疆医科大学第二附属医院、石河子大学医学院第一附属医院口腔门诊且经RCD/TMD双轴诊断确诊为TMD病人350例设为观察组,选取上述三家医院就诊且确诊无TMD的正常人群299例设为对照组,使用口腔健康影响程度量表(OHIP-14)进行问卷调查,对量表数据进行整理后使用统计软件SPSS17.0进行统计学分析。结果观察组和对照组的OHIP-14得分分别为(12.68±8.47)和(6.48±6.71)分。观察组14个条目得分均较对照组高,其中除条目2“味觉变差”、条目12“难以完成日常的工作”分数间的差异无统计学意义(P>0.05),其余12项条目分数间均差异有统计学意义(P<0.05)。将观察组病人按RCD/TMD轴Ⅰ诊断分类,其中筋膜痛病人得分(13.23±9.06)分,筋膜痛伴张口受限病人得分(14.00±7.90)分,可复性关节盘前移位病人得分(10.88±7.33)分,不可复性关节盘移位伴开口受限病人得分(14.42±8.43)分,不可复关节盘移位不伴开口受限病人得分(15.42±8.66)分,关节痛病人得分(14.91±7.33)分,骨关节炎病人得分(17.78±9.36)分,骨关节病病人得分(6.59±9.78)分。结论 TMD对病人的生活质量存在影响,对心理和生理的影响较明显。
Objective To evaluate the oral-related quality of life in patients with temporomandibular disorder (TMD) and to explore the relationship between psychological factors and TMD. Methods Three hundred and fifty patients with TMD diagnosed by the First Affiliated Hospital of Xinjiang Medical University, the Second Affiliated Hospital of Xinjiang Medical University and the First Affiliated Hospital of Shihezi University Medical School were diagnosed as TMD by RCD / TMD biaxial diagnosis. A total of 299 healthy subjects without TMD were selected as the control group. Questionnaire was conducted using the OHIP-14 questionnaire. Statistical data were analyzed using statistical software SPSS 17.0. Results The scores of OHIP-14 in observation group and control group were (12.68 ± 8.47) and (6.48 ± 6.71) points respectively. The scores of 14 items in the observation group were all higher than those in the control group. There was no significant difference among the scores of item 12 “hard to complete daily work ” except entry 2 “taste deterioration ” (P> 0.05) There were significant differences among the 12 items (P <0.05). The patients in the observation group were classified according to the RCD / TMD axis Ⅰ diagnostic criteria. Among them, the scores of fascia pain patients (13.23 ± 9.06) and fascia pain patients with limited mouth opening (14.00 ± 7.90) The patient score (10.88 ± 7.33) points, non-recapable disc displacement patients with open-limited patients score (14.42 ± 8.43) points, irrecoverable disc displacement patients with open restricted subjects (15.42 ± 8.66) points, joint Pain score (14.91 ± 7.33), osteoarthritis score (17.78 ± 9.36), and osteoarthritis score (6.59 ± 9.78). Conclusion The impact of TMD on the quality of life of patients, the impact on the psychological and physical more obvious.
