目的通过了解HBs Ag阳性孕产妇血清中HBV DNA含量和宫内感染率的关系,探讨不同护理方式对HBs Ag阳性孕产妇的产程护理与新生儿HBV的阳性率的影响以及孕产妇护理满意度的情况。方法应用荧光定量PCR法检测孕产妇及新生儿血清HBV DNA含量,酶联免疫(ELISA)法检测孕妇乙肝五项和新生儿HBs Ag;观察不同的护理方式对新生儿HBV阳性率的影响。结果被调查新生儿宫内感染率为9.28%。其中母亲HBV DNA≥107copy/ml和<107copy/ml的HBV宫内感染率分别为38.14%、2.64%,两者差异有统计学意义(P<0.05);母亲HBe Ag阳性和阴性的HBV宫内感染率分别为38.46%和2.44%,两者差异有统计学意义(P<0.05)。观察组孕产妇护理满意度为97.9%,明显高于对照组(81.3%),差异有统计学意义(P<0.05)。结论孕妇血清HBV DNA含量和HBe Ag水平与新生儿宫内感染相关。对HBV携带者孕产妇治疗的同时,加强产程观察并做好护理工作不仅可以降低新生儿感染率,还可以提高产科护理质量。
Objective To investigate the relationship between HBV DNA levels and the intrauterine infection rate in HBsAg-positive maternal serum and to explore the influence of different nursing methods on the birth-control of HBsAg-positive pregnant women and the positive rate of neonatal HBV and the satisfaction of maternal nursing Happening. Methods The levels of serum HBV DNA in pregnant women and newborns were detected by real-time fluorescence quantitative PCR. Five HBsAg in pregnant women and HBsAg in newborns were detected by enzyme linked immunosorbent assay (ELISA). The effect of different nursing methods on the positive rate of HBV in newborns was observed. Results The rate of intrauterine infection in neonates surveyed was 9.28%. The HBV intrauterine infection rates of mothers with HBV DNA≥107copy / ml and <107copy / ml were 38.14% and 2.64% respectively, with significant difference between the two groups (P <0.05); HBe Ag positive and negative The infection rates were 38.46% and 2.44%, respectively, with significant difference (P <0.05). The satisfaction rate of maternity nursing in observation group was 97.9%, which was significantly higher than that in control group (81.3%), the difference was statistically significant (P <0.05). Conclusion The serum HBV DNA level and HBe Ag level in pregnant women are related to neonatal intrauterine infection. While treating pregnant women with HBV carriers, strengthening labor observation and nursing work can not only reduce the neonatal infection rate, but also improve the quality of obstetric care.
