目的探讨老龄对糖耐量正常人群血糖和胰岛β细胞功能的影响。方法采用横断面多中心整群随机抽取新疆地区居民2117名,采用胰岛β细胞功能指数(HOMA-β)、早期相胰岛素分泌(AUCins/glu30min)、总胰岛素分泌(AUCins/glu120min)、早期相胰岛素处置指数(DI30min)及总胰岛素处置指数(DI120min)评估胰岛细胞功能,及胰岛素抵抗指数(HOMA-IR)评估IS。结果 (1)随年龄增长,FPG(P=0.004)和2hPG(P<0.001)呈上升趋势,HOMA-β(P=0.039)、AUCins/glu120min(P=0.022)、DI120min(P=0.023)、DI30min(P<0.001)及AUCins/glu30min(P=0.006)均呈降低趋势。(2)Logistic多元回归分析显示,胰岛β细胞功能与低龄、高BMI水平、良好控制饮食、受教育程度高及少饮酒相关。结论随年龄增长,胰岛β细胞功能及对IR的代偿能力逐渐减弱,血糖水平轻度升高,生活方式可能对年龄相关的胰岛β细胞功能下降有影响。
Objective To investigate the effect of aging on blood glucose and pancreatic β-cell function in normal glucose tolerance population. Methods A total of 2117 residents of Xinjiang region were randomly selected from a cross-sectional multicenter population. HOMA-β, AUCins / glu30min, AUCins / glu120min, The index of treatment (DI30min) and the total insulin treatment index (DI120min) were used to evaluate the function of islet cells and the insulin resistance index (HOMA-IR) to evaluate IS. Results (1) With the increase of age, the FPG (P = 0.004) and 2hPG (P <0.001) showed an increasing tendency. The HOMA-β, AUCins / glu120min (P = 0.022), DI120min DI30min (P <0.001) and AUCins / glu30min (P = 0.006) showed a decreasing trend. (2) Logistic multivariate regression analysis showed that pancreatic β-cell function was associated with younger age, higher BMI, well-controlled diet, higher education and less alcohol consumption. Conclusions With the increase of age, the function of islet β cells and the compensatory ability of IR gradually weakened, and the blood glucose level increased slightly. The lifestyle may have an effect on the age-related decline of pancreatic β-cell function.
