目的检测超体质量男性精浆中细胞因子TNF-α、IL-6水平含量,探讨其水平改变与超体质量的相关性及其对精液质量各参数的影响。方法从上海交通大学医学院附属瑞金医院生殖医学中心门诊就诊的男性患者中选取25例为超体质量组;另选取正常健康男性25例为对照组。对两组精液质量进行分析,用ELISA法检测精浆中TNF-α、IL-6水平含量。结果对照组精液体积、前向运动精子及正常形态精子都明显高于超体质量组(P<0.01);对照组精液浓度高于超体质量组,但无统计学意义。对照组精浆IL-6、TNF-α水平明显著低于超体质量组(P<0.01)。精浆IL-6、TNF-α均与体质量指数(BMI)呈正相关性(P<0.01),前向运动精子、精液体积和正常形态精子均与BMI呈负相关(P<0.01),TNF-α、IL-6与正常形态精子之间呈负相关(P<0.05)。结论男性超体质量患者精浆TNF-α、IL-6水平显著增加,其产生的毒性作用可引起精子形态和功能的改变,继而导致男性不育的发生。检测精浆中TNF-α、IL-6水平可为临床分析超体质量或肥胖所导致男性不育的病理机制提供重要的诊断依据。
Objective To detect the levels of cytokines TNF-α and IL-6 in the supernatant of male seminal plasma and to explore the correlation between the levels of TNF-α and IL-6 in seminal plasma and the effects on the semen parameters. Methods Twenty-five male patients selected as out-of-body mass from outpatient clinics of Reproductive Medicine Center affiliated to Ruijin Hospital of Shanghai Jiao Tong University School of Medicine were selected as the super-body mass group. Another 25 normal healthy men were selected as the control group. The semen quality of the two groups was analyzed, and the contents of TNF-α and IL-6 in seminal plasma were detected by ELISA. Results In the control group, semen volume, forward motility sperm and normal morphology sperm were significantly higher than those in the super-quality group (P <0.01). The semen concentration in the control group was higher than that in the super-quality group, but there was no statistical significance. The levels of IL-6 and TNF-α in the seminal plasma in the control group were significantly lower than those in the super-quality group (P <0.01). The seminal plasma IL-6 and TNF-α were positively correlated with body mass index (BMI) (P <0.01). There was a negative correlation between forward sperm motility, sperm volume and normal morphology sperm and BMI There was a negative correlation between -α, IL-6 and normal morphology (P <0.05). CONCLUSIONS: The levels of TNF-α and IL-6 in seminal plasma of male patients with hypersensitivity are significantly increased. The toxic effects of these drugs may cause changes in the morphology and function of sperm, leading to male infertility. Detection of seminal plasma TNF-α, IL-6 levels for clinical analysis of excess weight or obesity caused by male infertility pathological mechanisms provide an important basis for diagnosis.
