目的:探讨急性脑梗死(acute cerebral infarction,ACI)患者血清血管内皮生长因子(vascular endotheliaI growth factor,VEGF)及肿瘤坏死因子α(tumor necrosis factorα,TNF-α)浓度的动态变化及其临床意义。方法:急性脑梗死患者42例于入院24h、72h、7d、14d抽取静脉血,采用ELISA双抗体夹心法检测其血清VEGF及TNF-α含量。并分析血清VEGF及TNF-α的含量与病情、梗死面积的关系。同时以38例健康者为对照组。结果:入院24h、72h、7d、14d后,血清VEGF及TNF-α浓度均高于正常对照组,差异具有显著性(P<0.01),均于入院第7d达高峰,第14d下降,但仍未降至正常水平。大梗死灶组中血清VEGF及TNF-α含量高于中、小面积梗死灶组(P<0.01-0.05)。在入院24h、72h及7d,中梗死灶组中VEGF含量高于小梗死灶组(P<0.05)。病情重度者血清VEGF及TNF-α含量高于轻、中度者(P<0.01-0.05);在入院24h、72h及7d,病情中度组中VEGF含量高于轻度组(P<0.05);而血清TNF-α含量在轻、中度组中,差异无统计学意义(P>0.05)。结论:急性脑梗死患者血清VEGF及TNF-α升高,且血清VEGF及TNF-α含量与梗死面积和病情有关,梗死面积大者及病情重者血清VEGF及TNF-α含量高。
Objective: To investigate the dynamic changes of serum vascular endothelial growth factor (VEGF) and tumor necrosis factorα (TNF-α) in patients with acute cerebral infarction (ACI) and its clinical significance. Methods: Forty-two patients with acute cerebral infarction received venous blood at 24h, 72h, 7d and 14d after admission, and the levels of serum VEGF and TNF-αwere detected by ELISA double antibody sandwich method. And analyze the relationship between serum VEGF and TNF-α content and disease, infarct size. At the same time, 38 healthy people as the control group. Results: After admission for 24h, 72h, 7d and 14d, serum VEGF and TNF-α levels were significantly higher than those in normal control group (P <0.01), all of which reached the peak on the 7th day of admission and decreased on the 14th day Not down to normal level. Serum levels of VEGF and TNF-α in large infarct group were higher than those in medium and small infarct group (P <0.01-0.05). At 24 h, 72 h and 7 d after admission, the content of VEGF in middle infarction group was higher than that in the small infarction group (P <0.05). Serum levels of VEGF and TNF-αwere significantly higher than those in mild and moderate severe cases (P <0.01-0.05). The levels of VEGF in moderate disease group were higher than those in mild disease group at 24h, 72h and 7d after admission (P <0.05) ; While the level of serum TNF-α in mild and moderate group, the difference was not statistically significant (P> 0.05). CONCLUSION: Serum levels of VEGF and TNF-α are elevated in patients with acute cerebral infarction. The levels of serum VEGF and TNF-α are related to infarct size and severity. Serum levels of VEGF and TNF-α in patients with severe infarction and severe cases are high.
