目的 探讨联合检测肿瘤标志物在临床诊断中的意义。方法 1997年 3月至 1998年 5月由病理证实收治住院的恶性肿瘤患者 2 3 7例。其中肺癌 49例 ,大肠癌 3 6例 ,鼻咽癌 2 9例 ,肝癌 2 6例 ,乳腺癌 2 4例 ,淋巴瘤 15例 ,食道癌 13例 ,胃癌 9例 ,其他 3 6例。男性 15 2例 ,女性 85例。年龄在 14~ 78岁之间。经核医学放免技术同时测定血中CEA、SOD、SF、AFP、β2 MG值。 结果 SOD在胃癌中阳性率最高为 88.89% ( 8/9) ,总阳性率为 73 .42 % ( 174/2 3 7) ;AFP在肝癌中的阳性率最高为 73 .0 8% ( 19/2 6) ,总阳性率为 2 4.47% ( 5 8/2 3 7) ;β2 MG、SF在淋巴瘤中阳性率最高分别为 60 % ( 9/15 )、66.67% ( 10 /15 ) ,总阳性率分别为 40 .0 8% ( 95 /2 3 7) ,5 3 .5 9% ( 12 7/2 3 7) ;CEA在肺癌中的阳性率最高为 3 8.77% ( 19/4 9) ,总阳性率为 19.41% ( 4 6/2 3 7)。各肿瘤以阳性检出率最高的前三项组合除鼻咽癌外其阳性检出率均高于 90 % ,其中肝癌、食道癌、胃癌均达 10 0 %。结论 联合检测肿瘤标志物在临床诊断中意义更大。特别是合适的检测组合可起到互补作用从而明显地提高肿瘤的阳性检出率
Objective To explore the significance of combined detection of tumor markers in clinical diagnosis. Methods From March 1997 to May 1998, 237 patients with malignant tumors admitted to hospital were confirmed by pathology. There were 49 lung cancers, 36 colorectal cancers, 29 nasopharyngeal cancers, 26 liver cancers, 24 breast cancers, 15 lymphomas, 13 esophageal cancers, 9 gastric cancers, and 36 other cases. There were 152 males and 85 females. Age between 14 and 78 years old. By nuclear medicine radioimmunoassay, blood CEA, SOD, SF, AFP, and β2 MG values were measured at the same time. Results The highest positive rate of SOD in gastric cancer was 88.89% (8/9), and the total positive rate was 73.42% (174/2 3 7). The highest positive rate of AFP in liver cancer was 73.08% ( 19/ 26) The total positive rate was 24.47% (58/23); the positive rates of β2 MG and SF in lymphoma were 60% (9/15) and 66.67% (10/15) respectively. The positive rates were 40.8% (95/273), 53.5% (12 7/2 3 7), and the highest positive rate of CEA in lung cancer was 38.77% (19/49). The total positive rate was 19.41% (4 6/2 3 7). The positive rate of all three tumors with the highest positive detection rate was higher than 90% except nasopharyngeal carcinoma, of which liver cancer, esophageal cancer, and gastric cancer all reached 100%. Conclusion Joint detection of tumor markers is of greater significance in clinical diagnosis. In particular, suitable detection combinations can complement each other to significantly increase the positive detection rate of tumors.
