目的观察各种贫血血细胞CD55、CD59免疫表型相互变化关系,探讨其临床意义。方法用流式细胞仪技术检测各种贫血患者骨髓、外周血细胞CD55、CD59免疫表型。结果发现PNH患者血细胞膜表面与糖基磷脂酰肌醇(GPI)锚连蛋白有关抗原CD55、CD59免疫表达异常减低或缺乏,再障-PNH综合症CD55、CD59也呈不同程度降低。而且较传统常规溶血检测指标敏感。其它贫血血细胞膜表面CD55、CD59抗原表达正常。结论检测血细胞膜表面CD55、CD59GPI锚连蛋白分子表达可做PNH以及再障-PNH综合症确诊诊断和鉴别诊断的最敏感、最特异、最准确的检测指标,并对观察PNH发展、转化、治疗、愈后有重要指导价值。
Objective To observe the relationship between the immunophenotype changes of CD55 and CD59 in various kinds of anemia blood cells and to explore its clinical significance. Methods Flow cytometry was used to detect the immunophenotypes of CD55 and CD59 in bone marrow and peripheral blood of patients with various anemia. The results showed that the expression of anti-CD55 and CD59 antigens of glycosylphosphatidylinositol (GPI) on the surface of blood cells in PNH patients was abnormally reduced or absent, and the levels of CD55 and CD59 in patients with PNH were also decreased to some extent. And more sensitive than conventional hemolysis detection indicators. Other anemic blood cell membrane surface CD55, CD59 antigen expression is normal. Conclusion Detecting the expression of CD55 and CD59GPI ankyrin on the surface of blood cell membrane can be used as the most sensitive, specific and accurate detection index for diagnosis and differential diagnosis of PNH and aplastic anemia-PNH syndrome.
