目的探讨经直肠超声引导下前列腺穿刺活检术的教学方法,缩短学习曲线,减少学习期手术相关问题的发生。方法将完全未接触经直肠超声操作及穿刺相关操作的具有2~3年泌尿外科工作经验的6名住院医师随机分为两组:实验组和对照组,每组3名。两组均进行经直肠超声引导下的12针前列腺穿刺活检术。实验组采用“组块化分步学习”的培训模式;对照组采用传统的讲解+操作示教+全过程独立操作的学习方法。14d后对两组学员进行操作考试并进行针对该培训的问卷调查。结果 14d后,实验组全部3名学员均能顺利进行经直肠超声引导下的12针前列腺穿刺活检操作,而对照组只有1名学员能顺利规范完成。问卷调查显示,实验组3名学员均感觉该培训过程有趣而且不难,在实际操作过程中也没有出现紧张的情况;而对照组中只有1名学员感到操作过程较容易,另外2名学员感觉学习过程枯燥乏味,操作掌握难度较大,在实际患者身上操作过程中容易紧张、遗忘。结论经直肠超声引导下的12针前列腺穿刺活检术“组块化分步学习”的培训过程能够极大地提高学员的学习效率、增强学员的学习兴趣,循序渐进的培训过程使该操作的学习过程变得简单有趣。
Objective To explore the teaching methods of transrectal ultrasound-guided prostatic biopsy to shorten the learning curve and reduce the incidence of surgery-related problems. Methods Six residents with two to three years of urological work experience who had no contact with transrectal ultrasound operation and puncture related operation were randomly divided into two groups: experimental group and control group, with 3 in each group. Both groups underwent transrectal ultrasound guided 12-pin prostate biopsy. The experimental group adopted the training mode of “block-wise step-by-step learning”; the control group used the traditional method of teaching + operation teaching + the whole process of independent operation. After 14 days, two groups of students were taken operation tests and questionnaires for the training were conducted. Results After 14 days, all three participants in the experimental group were able to successfully undergo transrectal ultrasound-guided 12-pin prostatic biopsy while only 1 trainee in the control group could successfully complete the procedure. The questionnaire survey showed that all three participants in the experimental group felt that the training process was fun and not difficult, and that no tension occurred during the actual operation. Only one student in the control group felt that the operation was easier and the other two students felt that The learning process is boring, difficult to grasp the operation, the actual patient in the operation process is easy to stress, forgotten. Conclusion The 12-gauge prostate biopsy under the guidance of rectal ultrasound can improve the learning efficiency of trainees and enhance students’ interest in learning. The gradual training process enables the learning of this operation The process becomes simple and interesting.
