目的探讨超声造影(CEUS)技术诊断前列腺癌的准确率。方法对临床拟诊26例前列腺癌患者(共34个结节)进行CEUS检查,观察病灶造影后增强强度,并与对侧或周围正常组织对比,依据增强强度不同分为高增强、等增强、低增强和无增强四种不种的增强方式,结合时间强度曲线(TIC)判断病灶性质。结果经穿刺活检病理诊断30个结节为前列腺癌,4个为前列腺增生结节。癌结节中有25个呈高增强,且强度明显高于正常组织,而所有增生结节与正常组织几乎同时增强,强度与正常组织相当。结论CEUS能较为客观正确地评价前列腺内病灶的血流情况,可以引导进行穿刺活检;TIC可定量分析病灶的增强强度及方式,有助于前列腺癌的早期诊断。
Objective To investigate the diagnostic accuracy of contrast-enhanced ultrasound (CEUS) in the diagnosis of prostate cancer. Methods Twenty-six patients with prostate cancer (34 nodules in total) were enrolled in this study. CEUS examination was performed to observe the enhancement intensity after focal lesion and compared with contralateral or surrounding normal tissues. According to the enhancement intensity, CEUS was divided into high enhancement, Low enhancement and no enhancement four kinds of non-kind of enhancement, combined with the time intensity curve (TIC) to determine the nature of the lesion. Results Thirty nodules were diagnosed as prostate cancer by puncture biopsy and four were benign prostatic hyperplasia. 25 of cancerous nodules showed high enhancement and the intensity was significantly higher than that of normal tissues, while all the hyperplastic nodules were almost simultaneously enhanced with normal tissues, and the intensity was similar to that of normal tissues. Conclusion CEUS can objectively and correctly evaluate the blood flow in the prostatic intrahepatic lesion and guide the biopsy. TIC can quantitatively analyze the enhanced intensity and the way of the lesion, which is helpful for the early diagnosis of prostate cancer.
