目的探讨全身麻醉下神经内镜引导经鼻蝶入路切除术对垂体瘤患者的治疗效果。方法选取2014年5月至2015年7月朝阳市中心医院收治的87例垂体瘤患者作为研究对象,按随机数字表法将其分为对照组(43例)和观察组(44例)。对照组患者予以经颅手术治疗,观察组患者实施全身麻醉下神经内镜引导经鼻蝶入路切除术,比较两组患者治疗效果。结果观察组患者治疗的总有效率明显高于对照组,差异有统计学意义(P<0.05);观察组患者术中出血量明显少于对照组,手术时间、住院时间均明显短于对照组,差异均有统计学意义(均P<0.05);对照组患者并发症发生率为18.6%,观察组患者并发症发生率4.5%,观察组并发症发生率明显低于对照组,差异有统计学意义(P<0.05);观察组患者治疗后生理功能、身体疼痛、总体健康、活力、情感职能、心理健康评分均明显高于对照组,差异均有统计学意义(均P<0.05)。结论全身麻醉下神经内镜引导经鼻蝶入路切除术治疗垂体瘤临床效果显著。
Objective To investigate the therapeutic effect of endoscopic nasolabial approach guided by general anesthesia on patients with pituitary adenoma. Methods Eighty-seven patients with pituitary adenoma who were treated in Chaoyang Central Hospital from May 2014 to July 2015 were selected as study subjects and divided into control group (43 cases) and observation group (44 cases) according to random number table method. The patients in the control group were treated by transcranial surgery. The patients in the observation group underwent endoscopic nasal endoscopic neurotomy under general anesthesia. The therapeutic effect was compared between the two groups. Results The total effective rate of observation group was significantly higher than that of control group, the difference was statistically significant (P <0.05); the bleeding volume of observation group was significantly less than that of control group, the operation time and hospitalization time were significantly shorter than those of control group , The differences were statistically significant (all P <0.05); the incidence of complications in the control group was 18.6%, the incidence of complications in the observation group was 4.5%, the incidence of complications in the observation group was significantly lower than the control group (P <0.05). The scores of physical function, body pain, general health, vitality, emotional function and mental health of the observation group were significantly higher than those of the control group after treatment (all P <0.05). Conclusion Under general anesthesia, neuroendoscope guided transsphenoidal approach for the treatment of pituitary tumor has a significant clinical effect.
