目的:研究多聚免疫球蛋白受体(pIgR)在鼻息肉组织中的表达水平,并初步探讨pIgR在鼻息肉发病机制中的作用。方法:选取36例鼻息肉患者的鼻息肉组织标本为实验组,选取同期行鼻内镜治疗的12例鼻中隔偏曲患者为对照组,取下鼻甲组织标本。术后行苏木精-伊红染色评估鼻息肉组织中嗜酸粒细胞(Eos)的浸润情况,并依据Eos的浸润程度分为嗜酸粒细胞性鼻息肉组(Eos CRSwNP组,21例)和非嗜酸粒细胞性鼻息肉组(non-Eos CRSwNP组,15例)。采用免疫组织化学法检测组织中pIgR和IgA的表达情况,实时定量聚合酶链反应(RT-PCR)法检测组织中pIgR、IgA、人类维甲酸孤儿受体(RORc)及叉头转录因子(Foxp3)mRNA的表达水平,统计分析鼻息肉组织中pIgR的表达及与Eos数量、辅助性T细胞(Th17细胞)及调节性T细胞(Treg细胞)转录因子的相关性。结果:pIgR在实验组中的表达显著低于对照组,差异有统计学意义(P<0.05);Eos CRSwNP组中pIgR的表达低于non-Eos CRSwNP组,差异有统计学意义(P<0.05)。IgA在实验组中的表达显著高于对照组,差异有统计学意义(P<0.05)。RT-PCR分析结果显示:实验组中pIgR mRNA和Foxp3mRNA的表达低于对照组(P<0.05),RORc mRNA和IgA mRNA的表达高于对照组(P<0.05)。实验组中pIgR mRNA与RORc mRNA的表达呈负相关(P<0.05,r=-0.79),与Foxp3mRNA的表达无相关性(P>0.05,r=0.36)。结论:pIgR在鼻息肉组织中的表达下调,可能在鼻息肉的发生和发展中起着重要作用。
Objective: To investigate the expression of polyIgR in nasal polyps and to investigate the role of pIgR in the pathogenesis of nasal polyps. Methods: Nasal polyp samples of 36 patients with nasal polyps were selected as the experimental group. 12 patients with nasal septal deviation treated by endoscopic sinus surgery were selected as the control group, and the turbinate tissue specimens were removed. Postoperative hematoxylin-eosin staining to evaluate the infiltration of eosinophilic granulosa cells (Eos) in nasal polyps, and according to the infiltration of Eos into eosinophilic nasal polyp group (Eos CRSwNP group, 21 cases) And non-eosinophilic nasal polyp group (non-Eos CRSwNP group, 15 cases). Immunohistochemistry was used to detect the expression of pIgR and IgA in tissues. Real-time quantitative polymerase chain reaction (RT-PCR) was used to detect the expression of pIgR, IgA, human retinoic acid orphan receptor (RORc) and forkhead transcription factor Foxp3 ) mRNA in nasal polyps, and to analyze the correlation between the expression of pIgR and the number of Eos, the transcription of Th17 cells and Treg cells. Results: The expression of pIgR in experimental group was significantly lower than that in control group (P <0.05). The expression of pIgR in Eos CRSwNP group was lower than that in non-Eos CRSwNP group (P <0.05) ). IgA expression in the experimental group was significantly higher than the control group, the difference was statistically significant (P <0.05). The results of RT-PCR showed that the expression of pIgR mRNA and Foxp3 mRNA in the experimental group was lower than that in the control group (P <0.05), and the expression of RORc mRNA and IgA mRNA was higher than that in the control group (P <0.05). There was a negative correlation between pIgR mRNA and RORc mRNA expression (P <0.05, r = -0.79) in the experimental group, but no correlation with the expression of Foxp3 mRNA (P> 0.05, r = 0.36). Conclusion: The down-regulation of pIgR in nasal polyps may play an important role in the occurrence and development of nasal polyps.
