目的评价激光光纤介入内照射5-氨基酮戊酸盐酸盐-光动力疗法(OFI-ALA-PDT)和传统外照射5-氨基酮戊酸盐酸盐-光动力疗法(ALA-PDT)治疗兔耳痤疮模型的疗效,并对比两种方法的不良反应。方法参照美国皮肤病学会制定的Kligman法造模16只兔耳痤疮模型,采用煤焦油外涂和痤疮丙酸杆菌接种致兔耳发生痤疮样皮损,于皮损处外敷20%ALA后封包,每只兔耳沿长轴分为两部分,分别OFI-ALA-PDT和ALA-PDT疗法治疗,观察两种方案治疗兔耳痤疮样炎性丘疹的疗效和不良反应。结果家兔耳廓涂煤焦油2周后肉眼可见兔耳毛囊开口粉刺样发疹、接种痤疮丙酸杆菌处有痤疮样炎性丘疹,组织病理检查示其病变与人类痤疮炎性丘疹相似。治疗后第2、5、10d,肉眼观察光纤介入内照射组较传统外照射组皮疹消退快;治疗后第30d,肉眼观察两组疗效相似。在治疗后各个时间点肉眼观察,光纤介入内照射组副反应明显较传统外照射组少。结论 OFI-ALA-PDT疗法治疗兔耳痤疮模型可取得同传统外照射ALA-PDT疗法类似的疗效,且有更高的安全性和耐受性。
Objective To evaluate the effects of laser fiber interventional injections of 5-aminolevulinic acid hydrochloride-photodynamic therapy (OFI-ALA-PDT) and traditional external 5-aminolevulinic acid hydrochloride-photodynamic therapy (ALA-PDT) Rabbit ear acne model of efficacy, and compare the two methods of adverse reactions. Methods According to the method of Kligman established by the American College of Dermatology, 16 rabbits’ ear acne models were made. Acne-like skin lesions were induced by topical application of coal tar and inoculated with Propionibacterium acnes. After 20% ALA encapsulation, Each rabbit ear along the long axis is divided into two parts, respectively, OFI-ALA-PDT and ALA-PDT therapy, to observe the two programs for the treatment of rabbit ear acne-like inflammatory papules efficacy and adverse reactions. Results Rabbits ear acuity of coal tar 2 weeks after the naked eye can be seen rabbit ear hair follicles acne-like rash, acne propionate vaccination at acne-like inflammatory papules, histopathological examination showed lesions and human acne inflammatory papules similar. On the 2nd, 5th, and 10th day after treatment, the macroscopic observation showed that the IRR group regressed faster than the traditional external irradiation group; on the 30th day after treatment, the two groups were observed with similar curative effect. The naked eye observation at various time points after treatment showed that the side reactions in the interventional irradiation with optical fiber were significantly less than those in the conventional external irradiation group. Conclusion OFI-ALA-PDT treatment of rabbit ear acne model can achieve the same traditional external ALA-PDT therapy efficacy, and have a higher safety and tolerability.
