目的:研究外周血中性粒细胞-淋巴细胞比(NLR)及血小板参数与老年脑出血患者预后的相关性。方法:回顾性分析2017年1月至2019年7月我院神经外科收治的80例脑出血患者临床资料,根据改良Rankin量表评分结果,将改良Rankin量表评分<3分的49例患者纳入预后良好组,评分为3~6分的31例患者纳入预后不良组,比较两组患者一般人口学资料、实验室指标包括血小板、血小板分布宽度(PDW)、血小板平均压积(MPV)、淋巴细胞绝对值(ALC)、中性粒细胞绝对值(ANC),经Logistic回归分析血小板参数、NLR对老年脑出血患者预后的影响。结果:预后不良组NLR、PDW、MPV水平分别为8.89±1.95、(16.75±4.32)%、(14.82±3.52)%,高于预后良好组3.13±0.21、(12.89±2.71)%、(10.78±1.62)%;血小板水平预后不良组(0.37±0.20)%,低于预后良好组(0.62±0.33)%(n t=20.564、4.918、6.969、4.918,均n P<0.001);Logistic回归分析结果显示,NLR、PDW、MPV水平升高,血小板水平下降均是造成老年脑出血患者预后不良的影响因素(n OR=2.759、2.606、1.756、0.095,均n P<0.001)。n 结论:NLR、PDW、MPV、血小板均与老年脑出血患者预后密切相关,可考虑将其作为老年脑出血患者预后的评估指标。“,”Objective:To investigate the correlation of peripheral blood neutrophil-lymphocyte ratio(NLR)and platelet parameters with the prognosis of elderly patients with cerebral hemorrhage.Methods:Clinical data of 80 patients with cerebral hemorrhage admitted to the neurosurgery department of our hospital from January 2017 to July 2019 were retrospectively analyzed.Based on modified Rankin scale scores, they were classified into the good prognosis group(n=49, with modified Rankin scale scores <3 points)and the poor prognosis group(n=31, with modified Rankin scale scores between 3-6 points). General demographic data and results on laboratory indexes including platelet(PLT), platelet distribution width(PDW), mean platelet volume(MPV), absolute lymphocyte count(ALC)and absolute neutrophil count(ANC)were compared between the two groups.Logistic regression analysis was performed to examine the effects of platelet parameters and NLR on the prognosis of elderly patients with cerebral hemorrhage.Results:NLR, PDW and MPV were higher and PLT was lower in the poor prognosis group than in the good prognosis group[(8.89±1.95) n vs. (3.13±0.21), (16.75±4.32) % n vs.(12.89±2.71) %, (14.82±3.52) % n vs. (10.78±1.62) %, (0.37±0.20) % n vs.(0.62±0.33) %, n t=20.564, 4.918, 6.969 and 4.918, all n P<0.001)]. Logistic regression analysis showed that increased levels of NLR, PDW and MPV and decreased levels of PLT were influencing factors for poor prognosis in elderly patients with cerebral hemorrhage(n OR=2.759, 2.606, 1.756 and 0.095, all n P<0.001).n Conclusions:NLR, PDW, MPV and PLT are closely correlated with the prognosis of elderly patients with cerebral hemorrhage and may be used as indexes for prognosis evaluation in these patients.
