目的:观察癫癎患者自然睡眠时相与癎样放电的关系,探讨脑电Holter 24 h监测即俗 称的动态脑电图(AEEG)在癫癎的诊断及分型中的应用价值。方法:回顾分析761例癫癎患者24 h AEEG监测资料,对发作间期清醒与睡眠期脑电图癎样放电的发作频率进行比较。结果:761例患者中 检出癎样放电553例,睡眠期检出495例(检出率89.5%),觉醒期检出309例(55.9%),癎样放电主要 出现于NREM睡眠Ⅰ-Ⅱ期;93例患者AEEG监测后发作类型得到了修正。结论:睡眠期癎样放电检 出率明显高于觉醒期,癫癎患者睡眠期癎样放电主要出现于NREMⅠ-Ⅱ期,AEEG有助于癫癎的诊 断及分型。
OBJECTIVE: To observe the relationship between spontaneous sleep phase and hippocampal discharge in epileptic patients and to explore the value of EEG Holter 24 h monitoring, commonly known as dynamic electroencephalogram (AEEG), in the diagnosis and classification of epilepsy. Methods: A retrospective analysis of 761 cases of epilepsy patients with 24 h AEEG monitoring data, the interval between awake and sleep episodes of EEG discharge frequency were compared. Results: Of the 761 patients, 553 cases were detected as discharge discharge, 495 cases were detected during sleep (89.5%), 309 cases (55.9%) were detected during awakening, ; 93 patients with AEEG monitoring after the attack type has been amended. Conclusion: The detection rate of 癎-like discharge in sleep stage is significantly higher than that in awakening stage. 癎-like discharge in sleep stage of epilepsy mainly occurs in NREMⅠ-Ⅱ stage. AEEG is helpful for the diagnosis and classification of epilepsy.
