目的比较20~39孕周胎儿脐血在琼脂糖电泳中血红蛋白A(Hb A)的相对含量。方法 2007年1月-2015年7月在广东省人民医院产科及门诊进行地中海贫血产前筛查的188例孕妇为研究对象,按孕周分为20~24周组、25~28周组、29~34周组、35~39周组。定量测定各区带血红蛋白(Hb)相对含量。结果在188例脐血样本中,α基因型异常25例,Hb Bart’s区带范围为0.65%~31.61%;β基因型异常25例。20周后胎血Hb A相对含量随孕周的增加而逐渐增加,差异有统计学意义(P<0.05)。结论 20~39孕周的胎儿脐血Hb A相对含量呈动态变化,对产前检查诊断胎儿地中海贫血具有重要的指导意义,有利于优生优育。
Objective To compare the relative content of hemoglobin A (Hb A) in fetal cord blood of 20-39 gestational weeks during agarose electrophoresis. Methods From January 2007 to July 2015, 188 pregnant women undergoing antenatal screening of thalassemia in obstetrics and gynecology department of Guangdong Provincial People’s Hospital were enrolled in this study. The gestational age was divided into 20-24 weeks group, 25-28 weeks group, 29 ~ 34 weeks group, 35 ~ 39 weeks group. Quantitative determination of the zone with hemoglobin (Hb) relative content. Results 188 cases of umbilical cord blood samples, α genotype abnormalities in 25 cases, Hb Bart’s band ranged from 0.65% to 31.61%; 25 cases of β genotype abnormalities. After 20 weeks, the relative content of Hb A in fetal blood increased gradually with the increase of gestational age, the difference was statistically significant (P <0.05). Conclusion The relative content of Hb A in fetal umbilical cord blood from 20 to 39 gestational weeks has a dynamic change, which is of great significance in prenatal diagnosis of fetal thalassemia and is beneficial to prenatal and postnatal care.
