目的探讨分析依托咪酯复合舒芬太尼在小儿喉罩放置中的麻醉效果。方法 68例需要喉罩放置麻醉患儿,随机分成A组(36例)和B组(32例)。两组患儿均使用0.3μg/kg舒芬太尼缓慢注入诱导麻醉,然后A组以0.3 mg/kg剂量注入依托咪酯乳剂,B组以2 mg/kg剂量注入丙泊酚,放置喉罩均采用反转法。对两组患儿血压(BP)、心率(HR)、血氧饱和度(Sp O2)变化,以及屏气、呛咳、苏醒躁动、呕吐等不良反应进行对比分析。结果 A组三次的心率(HR)分别为(141.2±12.3)次/min、(138.1±12.7)次/min、(140.1±13.1)次/min,BP分别为(104.8±8.8)mm Hg(1 mm Hg=0.133 k Pa)、(102.8±6.8)mm Hg、(103.3±5.7)mm Hg,Sp O2分别为(94.8±3.7)%、(93.8±3.9)%、(94.2±3.4)%;B组HR三次分别为(142.3±13.8)次/min、(123.6±11.3)次/min、(126.2±11.8)次/min,BP分别为(106.8±9.0)mm Hg,(94.8±7.5)mm Hg,(94.2±7.7)mm Hg,Sp O2分别为(94.7±3.8)%,(90.1±2.9)%,(90.8±3.3)%。统计结果显示A组麻醉效果较B组稳定。在麻醉不良反应中A组较B组更少发生屏气和呛咳。结论依托咪酯复合舒芬太尼方法优于丙泊酚联合舒芬太尼,其麻醉效果更加稳定,且不良反应少。该方法安全有效值得推广应用。
Objective To investigate the anesthetic effects of etomidate and sufentanil in children with laryngeal mask placement. Methods Sixty-eight patients undergoing laryngeal mask placement were randomly divided into A group (36 cases) and B group (32 cases). Anesthesia was induced by slowly instillation of 0.3 μg / kg sufentanil in both groups, followed by injection of etomidate emulsion at 0.3 mg / kg in group A and propofol at 2 mg / kg in group B, and laryngeal mask Are used reverse method. The changes of blood pressure (BP), heart rate (HR), blood oxygen saturation (Sp O2) and adverse reactions such as breath holding, coughing, restlessness and vomiting were compared between the two groups. Results The heart rate (HR) of three times in group A were (141.2 ± 12.3) / min, (138.1 ± 12.7) / min, (140.1 ± 13.1) / min, BP were (104.8 ± 8.8) mm Hg (94.8 ± 3.7)%, (93.8 ± 3.9)%, (94.2 ± 3.4)%, respectively; B (102.8 ± 6.8) mm Hg and (142.3 ± 13.8) times / min, (123.6 ± 11.3) times / min and (126.2 ± 11.8) times / min respectively in the three groups, with BP of (106.8 ± 9.0) mm Hg, (94.8 ± 7.5) mm Hg , (94.2 ± 7.7) mm Hg and Sp O2 were (94.7 ± 3.8)%, (90.1 ± 2.9)% and (90.8 ± 3.3)%, respectively. Statistics show that the anesthesia in group A is more stable than in group B. Anesthetic adverse reactions in group A occurred less breath-hold and cough than in group B. Conclusion The combination of etomidate and sufentanil is better than propofol and sufentanil in anesthesia, and the anesthetic effect is more stable with less adverse reactions. The method is safe and effective and worth popularizing and applying.
