目的本研究通过血清胸苷激酶(TK1)、鳞状细胞癌相关抗原(SCC-Ag)的检测来提高喉癌的诊断率,并研究他们与临床分期及病理分级的相关性。方法采集80例喉癌受检者、30例健康人群作为对照组;所有患者的血清标本均采用免疫印迹增强化学发光法(ECLA)检测TK1,采用微粒子酶免疫分析(MEIA)检测SCC-Ag。结果喉癌患者血清TK1、SCC-Ag的含量明显高于健康对照组,有统计学意义(P<0.01)。血清TK1及SCC-Ag联合检测阳性率为88.5%,明显高于各个单项检测的阳性率(分别为61.2%、69.3%,P<0.01)。血清TK1及SCC-Ag含量与喉癌的分期有关,其中均随喉癌的分期增高而增高,有统计学意义(P<0.05)而与喉癌的病理分级无相关性(P>0.05)。结论 TK1、SCC-Ag在喉癌中的检出率明显高于健康人群;均随喉癌的临床分期的升高而升高;在喉癌的检测中TK1的敏感性较高,特异性较低;而SCC-Ag的特异性较高,敏感性较低,二者联合检测可以提高喉癌检出的特异性及敏感性,从而可以提高喉癌的早期诊断率。
Objective To improve the diagnostic rate of laryngeal carcinoma by the detection of serum thymidine kinase (TK1) and squamous cell carcinoma associated antigen (SCC-Ag) and to study their correlation with clinical stage and pathological grade. Methods 80 cases of laryngeal cancer and 30 healthy subjects were collected as control group. TK1 was detected in serum samples of all patients by immunoblot enhanced chemiluminescence (ECLA) assay, and SCC-Ag was detected by microparticle enzyme immunoassay (MEIA). Results The serum levels of TK1 and SCC-Ag in patients with laryngeal carcinoma were significantly higher than those in the healthy control group (P<0.01). The positive rate of combined detection of serum TK1 and SCC-Ag was 88.5%, which was significantly higher than the positive rate of each individual test (61.2%, 69.3%, respectively, P<0.01). The serum TK1 and SCC-Ag levels were related to the stage of laryngeal cancer, and all of them increased with the stage of laryngeal cancer. There was statistical significance (P<0.05) but no correlation with the pathological grade of laryngeal cancer (P>0.05). Conclusions The detection rates of TK1 and SCC-Ag in laryngeal cancer are significantly higher than those in healthy people; they all increase with the clinical stage of laryngeal cancer; the sensitivity of TK1 in laryngeal cancer detection is higher, and the specificity is higher. Low; SCC-Ag has higher specificity and lower sensitivity. The combination of the two can improve the specificity and sensitivity of laryngeal cancer detection, which can improve the early diagnosis of laryngeal cancer.
