目的探讨急性颅脑损伤(TBI)患者垂体前叶功能紊乱的危险因素。方法符合入选标准的82例TBI患者,于伤后~3 d采取静脉血,检测垂体激素水平,与正常值作对照;同时将对垂体激素分泌有影响的14项可能因素进行统计学分析。结入选的TBI患者中,82.93%发生垂体前叶激素分泌紊乱,经Logistic回归分析表明,GCS评分3~8分和中线结构移位≥0.5 cmTBI患者伤后前垂体功能紊乱的危险因素(P<0.05)。结论 TBI后垂体前叶激素分泌紊乱发生率极高,脑损伤程度和中线结构位是发生TBI后垂体前叶激素分泌紊乱的重要危险因素。
Objective To investigate the risk factors of anterior pituitary dysfunction in patients with acute craniocerebral injury (TBI). Methods Totally 82 TBI patients were enrolled in this study. Venous blood was collected at ~ 3 days after injury and the level of pituitary hormone was measured to compare with the normal value. At the same time, 14 possible factors influencing the secretion of pituitary hormone were statistically analyzed. Logistic regression analysis showed that 82.93% of patients with TBI were involved in the disorder of pituitary dysfunction. The risk factors of anterior pituitary dysfunction after GCS score 3-8 and midline structural displacement≥0.5 cmTBI patients (P < 0.05). Conclusions The incidence of anterior pituitary hormone disorder is very high after TBI. The degree of brain injury and median structure are the important risk factors for the disorder of anterior pituitary hormone secretion after TBI.
