目的 利用经胸冠脉血流显像技术 (CDCFI)观测X综合征患者的左冠状动脉前降支血流 ,评价冠脉血流的变化及冠脉血流储备 (CFR)。方法 利用CDCFI结合潘生丁药物负荷分别测量 2 0例X综合征患者 (A组 )和 2 2例健康者 (B组 )的前降支中远端静息及最大充血状态时冠脉舒张期峰值血流速度 (CPV)及减速时间 (DT) ,以最大充血状态与静息时的比值计算冠脉血流储备 (CFR) ,并对时间参数DT以R R间期进行校正。结果 静息时两组的CPV无明显差别 (P >0 .0 5 ) ;A组DT明显低于B组 (P <0 .0 5 )。最大充血状态时 ,两组的CPV均明显增快 ,但A组增加的程度低于B组 (P <0 .0 0 1) ;A组的DT明显低于B组的DT (P <0 .0 5 )。两组的CFR值有显著性差异(P <0 .0 0 1)。结论 经胸冠脉血流显像是一种能够准确有效测量X综合征的冠脉血流储备的无创性检测方法 ,对临床确诊X综合征及评估疗效具有重要指导意义
Objective To observe the changes of coronary blood flow and coronary blood flow reserve (CFR) by using transthoracic coronary flow imaging (CDCFI) to observe the left anterior descending coronary artery in patients with X syndrome. Methods CDCFI and dipyridamole drug load were used to measure the peak coronary diastolic blood flow at the mid-and-distal resting and maxillary congenital anterior descending in 20 patients with syndrome X (A group) and 22 healthy subjects (group B) Flow rate (CPV) and deceleration time (DT), coronary flow reserve (CFR) was calculated as the ratio of maximal congestive state to resting time, and the time parameter DT was corrected for RR intervals. Results There was no significant difference in CPV between the two groups at rest (P> 0.05). The DT in group A was significantly lower than that in group B (P <0.05). The maximal congestive state, CPV were significantly increased in both groups, but the degree of increase in group A was lower than that in group B (P <0.01). The DT in group A was significantly lower than that in group B (P <0. 0 5). There was a significant difference in CFR between the two groups (P <0.001). Conclusion Transthoracic coronary flow imaging is a noninvasive method to accurately and effectively measure coronary blood flow reserve of syndrome X. It has important guiding significance for clinical diagnosis of syndrome X and evaluation of curative effect
