目的了解长沙市某医院近5年AIDS住院患者感染菌群分布的特点及其变化情况。方法利用生物梅里埃ATB New 1.1 beta软件对该院2010-2014年AIDS住院患者血液和脑脊液(CSF)细菌培养阳性菌株鉴定结果进行回顾性统计分析。结果 2010-2014年该院AIDS住院患者血液和CSF细菌培养年阳性率分别为10.81%、2.90%(2010年);17.90%、5.14%(2011年);19.36%、3.67%(2012年);14.37%、3.00%(2013年)和17.28%、4.13%(2014年)。其中血培养阳性率不同年间差异有统计学意义(χ~2=16.23,P<0.05)。脑脊液培养阳性率不同年间差异无统计学意义(χ~2=4.97,P>0.05)。血培养前3位病原菌构成比依次为马尔尼菲青霉菌(49.68%~58.26%),新型隐球菌(10.06%~18.39%),葡萄球菌属(14.68%~30.82%);CSF培养前2位病原菌依次为新型隐球菌,葡萄球菌属。血液和CSF培养病原菌中,葡萄球菌属构成比波动性大,但未呈现逐年上升趋势,差异无统计学意义(χ~2=9.38,P=0.05;χ~2=2.42,P>0.05)。结论 2010-2014年长沙市某医院AIDS住院患者血液和CSF培养病原菌主要以真菌为主,细菌主要是葡萄球菌属,其他均为散发形式分布,病原菌年总阳性率有一定波动性,未见明显的逐年上升或下降趋势。
Objective To understand the characteristics and changes of bacterial flora distribution in AIDS inpatients in a hospital in Changsha in recent 5 years. Methods Bioinformatics ATB New 1.1 beta software was used to retrospectively analyze the identification results of positive bacterial cultures of blood and cerebrospinal fluid (CSF) from 2010 to 2014 in the hospital from 2010 to 2014. Results The positive rate of blood and CSF culture in hospitalized patients with AIDS from 2010 to 2014 were 10.181%, 2.90% (2010), 17.90%, 5.14% (2011), 19.36%, 3.67% (2012) respectively. 14.37%, 3.00% (2013) and 17.28%, 4.13% (2014). The positive rate of blood culture in different years was statistically significant (χ ~ 2 = 16.23, P <0.05). The positive rate of cerebrospinal fluid culture in different years was no significant difference (χ ~ 2 = 4.97, P> 0.05). The proportions of the three pathogenic bacteria before blood culture were as follows: Penicillium marneffei (49.68% -58.26%), Cryptococcus neoformans (10.06% -18.39%) and Staphylococcus (14.68% -30.82% Pathogenic bacteria followed by Cryptococcus neoformans, Staphylococcus. Staphylococcus was more volatile in blood and CSF-producing pathogens, but did not show a trend of increasing year by year. The difference was not statistically significant (χ ~ 2 = 9.38, P = 0.05; χ ~ 2 = 2.42, P> 0.05). Conclusion From 2010 to 2014, the pathogenic bacteria of blood and CSF in AIDS inpatients in a hospital in Changsha City were mainly fungi, and the bacteria were mainly Staphylococcus. The others were in the form of sporadic distribution. The total positive rate of pathogenic bacteria had some fluctuation in 2010-2014. Year after year rise or fall trend.
