目的 探讨细胞因子基因型多态性对肾脏移植后急性排斥反应发生率的影响。 方法采用序列特异引物聚合酶链反应 (PCR SSP)法对 115例肾移植受者和 2 4名健康对照者的细胞因子肿瘤坏死因子 (TNF) α ,转化生长因子 (TGF) β1,白细胞介素 (IL) 6和IL 10基因型进行测定 ,探讨其细胞因子基因型对肾脏移植后急性排斥反应发生率的影响。 结果 在 115例肾移植受者及 2 4名健康对照者中 ,TNF α和IL 10低产生型基因型者分别为 88 7% (10 2 / 115 ) ,87 5 % (2 1/ 2 4 )和 80 % (92 / 115 ) ,75 % (18/ 2 4 ) ,占明显优势。而TGF β1和IL 6的基因型均是以高产生型占明显优势 ,分别为 79 1%(91/ 115 ) ,10 0 % (115 / 115 )和 6 6 7% (16 / 2 4 ) ,10 0 % (2 4 / 2 4 )。 115例肾移植受者中 ,在移植后 6个月内共有 2 6例发生了急性排斥。其中TNF α高产生基因型和IL 10的中、高产生基因型受者的急性排斥发生率分别为 5 3 8% (7/ 13)和 4 3 5 % (10 / 2 3) ,显著高于相应的低产生基因型受者的 18 6 % (19/ 10 2 ,χ2 =8 17)和 17 4 % (16 / 92 ,χ2 =7 16 ) ,差异有非常显著意义 (P值均 <0 0 1)。 结论 肾移植受者和正常健康人群的TNF α和IL 10基因型是以低产生型为主 ;TGF β1和IL 6的基因型是以高产生?
Objective To investigate the influence of cytokine genotypes on the incidence of acute rejection after kidney transplantation. Methods Serum levels of tumor necrosis factor (TNF) α, transforming growth factor (TGF) β1, interleukin (IL) 6 and IL 10 genotypes were determined to investigate the influence of cytokine genotypes on the incidence of acute rejection after kidney transplantation. Results Among the 115 renal transplant recipients and 24 healthy controls, the TNFα and IL 10 low-producing genotypes were 88 7% (10 2/115), 87 5% (2 1/24) And 80% (92/115) and 75% (18/2 4), respectively. However, the genotypes of TGFβ1 and IL6 were significantly higher in high-yielding genotypes, which were 79 1% (91/115), 100% (115/115) and 66.7% (16/2 4) respectively, 10 0% (2 4/2 4). Of the 115 renal transplant recipients, 26 received acute rejection within 6 months of transplantation. The acute rejection rates of moderate and high genotype recipients with TNFα-high genotype and IL-10 were 53.8% (7/13) and 43.5% (10/23), respectively, which were significantly higher than There was significant difference between the corresponding low-genotype genotype recipients (18.6% (19/102; χ2 = 817) and 17.4% (16/92, χ2 = 716) 1). Conclusion The genotypes of TNFα and IL-10 in kidney transplant recipients and normal healthy people are mainly low-producing ones. The genotypes of TGF-β1 and IL-6 are high-producing.
