目的了解上海市虹口区15岁以上人群麻疹抗体水平和麻疹发病情况,为制定成人麻疹免疫策略提供依据。方法定量ELISA法检测本区及外来15岁以上人群的麻疹抗体水平,与微量细胞中和法检测结果进行比较,并对1998~2007年本区和外来16岁以上人群麻疹发病资料进行分析。结果本区和外来15岁以上人群麻疹抗体平均阳性率分别为96.83%和98.21%,麻疹抗体GMT平均水平分别为1217.86和1553.73IU/ml,本区各年龄组抗体GMT水平15~19岁组最低,40岁以上组最高;外来各年龄组间抗体GMT水平差异无统计学意义,15~19岁组和30~39岁组麻疹抗体水平显著高于本区人群;ELISA法与微量细胞中和法检测结果呈正相关,回归方程有统计学意义,15岁以上人群中仅有83.5%达到麻疹抗体保护性水平;近10年来,本区与外来16岁以上人群中麻疹发病比例有逐年增高的趋势,本区发病比例显著高于外来人群。结论为消除麻疹,有必要制定成人麻疹免疫策略,其中本区成人麻疹防制策略较外来成人更为紧迫。
Objective To understand the level of measles antibody and the incidence of measles in people over 15 years of age in Hongkou District, Shanghai, and to provide basis for formulating adult measles immunization strategy. Methods The level of measles antibody in the population aged 15 years and above in our district and outside of our country was detected by quantitative ELISA and compared with the result of the method of neutrophil neutralization test. The incidence of measles in this area and in the outside of the population aged 16 years and above was analyzed from 1998 to 2007. Results The average positive rate of measles antibody in this area and that of outside population over 15 years old was 96.83% and 98.21% respectively. The average GMT levels of measles antibody were 1217.86 and 1553.73 IU / ml, respectively. The antibody GMT level in all age groups in this area was the lowest , And the highest in the group over 40 years old. There was no significant difference in the antibody GMT levels among all age groups. The antibody levels of measles in 15 ~ 19 years old group and 30 ~ 39 years old group were significantly higher than those in the local area. ELISA and micro cell neutralization The test results showed a positive correlation, the regression equation was statistically significant, only 83.5% of the population over the age of 15 to measles antibody protection level; the past 10 years, the region and foreign population over the age of 16 measles incidence increased year by year trend, The incidence of this area was significantly higher than the foreign population. Conclusion In order to eliminate measles, it is necessary to formulate an adult measles immunization strategy. Among them, adult measles prevention measures in this area are more urgent than those of foreign adults.
